IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity is issued to Convergent Communications Services, Inc., Certificate No. 378, to authorize Convergent to provide resold local telecommunication services in the areas served by Qwest Corporation (formerly known as U S WEST Communications, Inc.) and GTE Northwest Incorporated. The Commission approved Convergent’s Certificate by Order No. 28389 issued June 5, 2000. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 28389.
