FIRST AMENDED
CERTIFICATE NO. 348
IT IS HEREBY CERTIFIED that this Amendment to the Certificate of Public Convenience
and Necessity, Certificate No. 348, is issued to CTC Telecom, Inc. This Amendment authorizes
CTC to provide Title 61 and Title 62 telecommunications services, including basic local exchange
services, as a competitive local exchange carrier to the inhabitants of the area set forth and described
in Certificate No. 348, made a part of this Certificate by reference, and to the inhabitants of the area
set forth and described within the service territories of U S WEST Communications, Inc. in the state
of Idaho. The Commission approved this amendment to CTC’s Certificate No. 348 by Order
No. 28059 issued June 8, 1999, in Case No. CTL-T-99-1. The Commission approved CTC’s
Certificate No. 348 by Order No. 27673 issued August 10, 1998. Order No. 28059 limits the areas in
which CTC is authorized to provide telecommunications services within the service territories of U S
WEST Communications, Inc. in the state of Idaho as fully described in U S WEST’s Certificate
No. 255, as amended.
This Certificate is predicated upon and is issued pursuant to the findings of the Commission
in Order No. 28059.
