Application for Commission Determination of Qualified Broadband Equipment for
the Year 2004
Pursuant to Commission Order No. 28784, in Case No. GNR-01-, Citizens
Telecommunications Company of Idaho submits the following information as an
application for Commission determination that the subject installed broadband
equipment qualifies for investment tax credit under Idaho Code ~ 63-30291.

Type of broadband service offered to the public: Asymmetrical Digital Subscriber Line
(ADSL)
Network transmission rate: Up to 512Kbps send and 3 Mbps receive

Inst$lIation dates: January 2004 - December 2004
Number and Percentage of customers: Broadband is now available to approximately
500 Frontier's Idaho customers. This is over 87% of the total customer base.
