Custer Telephone Broadband Senrices LLC ("CTBS" or "Applicant'), by and through its
attorneys, Cynthia A. Melillo PLLC, files this Applicafion for an Idaho Ptrblic Utilities
Commission ('Commission') order certi$ing that certain telecommunications equipnent is
eligible for the broadband infrastnrcture tax sredit authorized by Section 63-3029,Idaho Code.
In support of its Applicatio& CTBS states as follows:
l. CTBS is a provider of basic local exchange sendce, broadband
telecommunications senrice and other telecommunications senrices in Idaho.
2. During the calendar year 2021, CTBS made certain investrre,nts that constitute
*qualified broadband equipme,nt" within the meaning of Section 63-3029(D(3)O), Idaho Code.
Exhibit A, attached hereto, describes CTBS's qualified broadband equipment and contains the
infonnation and representations required by this Commission's Order No. 35297 in Case No.
GNR-T-21-10.
