Check Sheets. When a tariff (price sheet) filing is made with the IPUC, an
updated Check Sheet accompanies the filing. The Check Sheet lists the sheets
contained in the filing with a cross reference to the current revision number.
When new pages are added, the check sheet is changed to reflect the revision.
Numbering. Sheet numbers appear in the upper left corner of each page. Sheets
Are numbered sequentially. When a new sheet is added between sheets already
In effect, a decimal is added. For example, a new sheet added between sheets 6
and 7 would be 6.
Sheet revision numbers also appear in the upper left corner of each page. These
numbers are used to determined the most current sheet version on file with the
IPUc. For example, the 4th revised sheet 6 cancels the 3rd revised sheet 6.

The Company may deny or terminate a service or all services without prior notice
To the customer or applicant and without the customer s or applicant's permission
For one or more of the following reasons:
Dangerous Condition. A condition immediately dangerous or hazardous to life
physical safety, or property exists , or it is necessary to prevent a violation of
federal, sate or local safety of health codes.
Ordered to Terminate Service. The Company is ordered to terminate service by
any court, the IPUC ( the "Commission ), or any other duly authorized public
authority.
Service Obtained Illegally. The service(s) was (were) obtained, diverted or used
without the authorization or knowledge of the Company.
Customer Unable to be Contacted. The Company has tried diligently to meet the
Notice requirement ofIDAP A Rule 304, but has been unsuccessful in its attempt
to contact the customer affected.
Misrepresentation of Identity. The customer has misrepresented the customer
identity for purposed of obtaining telephone service and has no or an inadequate
security deposit on file with the company and has an outstanding bill exceeding
one hundred (100.00) dollars.

Service Offering
Calculation of Distance - Because the Company charges "flat" rates based on the
length of the call rather than Distance, there are no mileage sensitive products
applicable to this tariff.
Minimum Call Completion Rate - A customer can expect a completion rate of no
less than 90% during peak use periods for all services. The call completion
rate is calculated as the number of calls completed, including calls completed to
a busy or unanswered line, divided by the number of calls attempted.
Description of Services Offered Intrastate Long Distance Telecommunications
and Service consists of the furnishing of switched and dedicated telephone service
between points within the State of Idaho. Such service is available twenty-four
(24) hours a day, seven (7) days a week. All services are intended solely for the
purposes of conducting legal and ethical business or for the personal use of the
Company s residential customers.
Operator Service. The appropriate per minute rates as described in Section 7.
will apply, plus a call placement charge added to the first minute of each call as
shown in Section 7.4. An operator-dialed surcharge will be applied if necessary.
Direct Dial (One Plus).One plus service is a standard long distance service for
business and residential customers offered at a "discounted" rate (compared to
national long distance companies). Rates are described in Section 7., 7.

800 Service.800 Service is another standard long distance service provided to
businesses for the purposes of customer service or sales of products Residential

customers use 800 service to have friends or relatives call them free of charge.
Rates are described in Section 7.2.4, and 7.3.4.
3.4 Dedicated Service (Includes Outbound and 800).Dedicated Services is
a heavily discounted service for large-volume users of long distance and is used
by business customers only. Rates are described in Section 7., 7., 7.3.2
and 7.
Calling Card. Calling card services enable a Customer to bill telephone calls to
his account via a card assigned by the Company. Rates are described in Section
7.2., 7.3 and 7.
6.4 Liability of Company.
6.4.Notice. The Company will give at least 10 days' notice to customers and the
Idaho PUC before increasing rates of other changes. The notice to customers
will be either individual notice or a public notice in the newspapers in the
Company s Idaho service areas.
6.4.2 Availability.All service is subject to availability of suitable facilities. The
Company reserves the right to limit the length of communications or to
discontinue services when necessary because of the lack oflocal facilities or other
transmission medium capacity of because of any causes beyond its control.
6.4.Limitation of Liability. Except as stated on this Section 6.4., the Company shall
have no liability for damages of any kind arising out of or related to events, acts
rights or privileges contemplated in this tariff. This tariff does not limit the
liability of the Company for willful misconduct.
The liability of the Company for damages resulting in whole or in part from or
arising in connection with the furnishing of this service under this tariff, including
but not limited to mistakes, omissions, interruptions, delays, errors or other

defects or misrepresentations shall not exceed an amount equal to five time the
initial minute charge provided for under this tariff for the interstate long distance
call for the period during which the call was affected. No other liability in any
event shall attach to the Company.
The Company shall not be liable for any failure of performance hereunder due to
causes beyond its control, including but not limited to acts of God, fires, flood or
other catastrophes; any law, order, regulation, directive, action or request of the
United States Government, or any other government, including state and local
governments having jurisdiction over the Company, or of any department
agency, commission bureau, corporation or other instrumentality or anyone or
more of said governments, or of any civil or military authority; national
emergencies, insurrections, riots, wars or other labor difficulties.
The Company shall not be liable for any act or omission of any other entity
furnishing to the Customer facilities, equipment, or services used with the
Company s Long Distance Telecommunications Service. Nor shall the company
be liable for any damages or losses due to the failure or negligence of the
Customer or due to the failure of Customer-provided equipment, facilities or
services.
The Company shall be indemnified and saved harmless by the Customer from and
against all loss, liability, damages and expense, including reasonable counsel fees
due to claims for libel, slander, or infringement of copyright in connection with
the material transmitted over the Company s facilities; and any claim resulting
from any act or omission of the Customer ofpatron(s) of the Customer relating
to the use of the Company s facilities.
The included tariff language does not constitute a determination by the
Commission that a limitation of liability imposed by the Company should be
upheld in a court of law. Acceptance for filing by the Commission recognizes
that it is a court's responsibility to determine the validity of the exculpatory
clause.

Pricing
Explanation of Rates for Intrastate and Interstate Long Distance
Telecommunications Services
The rates for the Company Intrastate and Interstate Long Distance
Telecommunications Service will depend on the length of the call. Rates are "flat"
rated and apply to all Idaho calls. The Rate Schedules in this section represent the
maximum applied rates for Intrastate and Interstate services.
All services are billed in six-second to thirty-second minimum and six-second
rounding thereafter, except that calling-card services are billed in 30-second
minimum and six-second rounding thereafter.
