NOTICE OF DISCONTINUANCE OF
TELECOMMUNICATIONS SERVICES
On May 1,2011,Cordia Communications Corp.(“Cordia”)filed under Chapter 11 of the United States
Bankruptcy Code in the Middle District of Florida,Case Number 6:11-bk-06493.On July 14,2011,the
bankruptcy court approved an Asset Purchase Agreement between Birch Communications Inc.(“Birch”)
and Cordia that provides for the sale of certain assets and customers of Cordia.
The Asset Purchase Agreement does not include customers located in Idaho and Utah.
You are receiving this correspondence because effective October 13,2011,Cordia will cease providing
telecommunications services to you.To avoid disruption of service or loss of your telephone number it
is your responsibility to obtain a new telecommunications service provider before October 13,2011.
