This price list applies to the pay telephone and MTS services furnished by CfL, LLC (d/b/a CfL
Payphones), hereafter referred to as "Company or CfL Payphones" for calls which originate and
terminate within the State of Idaho. This price list is on file with the Idaho Public Utilities Commission
located at 472 W. Washington, Boise, Idaho. Copies may be inspected during normal business hours at the
company s principal place of business at 725 North Derby Lane, North Sioux City, SD.
Service is provided within the State of Idaho.
The "Company" provides pay telephone service, uses underlying carriers to provide Operator
Services, and other toll related functions. The "Company " underlying carrier charges its tariffed
rates and apprises the IPUC with timely price list updates. However, the "Company" does rate its
own 1 + toll traffic as provided in this price list. "Company" uses V & H coordinates established
by Bellcore NECA Tariff #4 to determine mileage between cities. "Company" does rate local
calls from its pay telephones at 25 cents per call.
The "Company" requires no deposit by the site provider and provides service as outlined in its
contract which also stipulates the terms and conditions for the removal of service.
Customer complaints and bill inquiries should be routed to the above number or address. The
premises equipment and in some cases the site provider offers toll free reporting of problems and
the equipment is labeled with pertinent reporting information.
The "Company" only installs equipment that adheres to the rules established by the IPUC, FCC
Telecommunications for the Disabled Act of 1982 and Title III of the Americans with Disabilities
Act of 1990.

Check Sheets. When a price sheet filing is made with the IPUC, an updated Check Sheet
accompanies the filing. The Check Sheet lists the sheets contained in the filing with a cross
reference to the current revision number. When new pages are added, the Check Sheet is
changed to reflect the revision.
Numbering. Sheet numbers appear in the upper left corner of each page. Sheets are
numbered sequentially. When a new sheet is added between sheets already in effect, a
decimal is added. For example, a new sheet added between 6 and 7 would be 6.
2.2.Sheet revision numbers also appear in the upper left corner of each page. These
numbers are used to determine the most current sheet version on file with the IPUC.
For example, the 4th revised sheet 6 cancels the 3rd revised sheet 6. Consult the
check sheet for the sheet currently in effect.
Paragraph Number sequence - There could be several levels of paragraph coding. Each
level of coding is subservient to its next higher level.

SERVICE OFFERING
CfL Payphones provides pay telephone service, using underlying carriers to provide
Operator Services, and other toll related functions. The underlying carrier charges its
tariffed rates and apprises the IPUC with timely price list updates. However, CfL
Payphones does rate its own 1 + toll traffic as provided in this price list and uses V & H
coordinates established by Bellcore NECA Tariff #4 to determine mileage between cities.
CfL Payphones rates local calls from its pay telephones at 25 cents per call.
CfL Payphones rates long distance calls from its pay telephones at 25 cents per minute
with a 4 minute minimum per call..
CfL Payphones only installs equipment that adheres to the rules established by the IPUC
FCC, Telecommunications for the Disabled Act of 1982 and Title III of the Americans
with Disabilities Act of 1990.
CfL Payphones' services are offered to the location owner on a contract by contract
basis.
CfL Payphones' services are offered to the transient public twenty-four hours a day, 7
days a week. However, these hours and services are dependent upon the business hours
established by the location where CfL Payphones' equipment is installed.
CfL Payphones will give at least 10 days notice to customers and the Idaho PUC before
increasing rates or other changes. The notice to customers will be either individual
notice via signage placed at each location or public notice placed in the newspaper in CfL
Payphones' Idaho service areas.
Responsibility and Liability of Use of Services
CfL Payphones' services may be used for any lawful purpose consistent with the
transmission and switching parameters of the telecommunications facilities utilized
in the provision of services.
The use of CfL Payphones' services to make calls which might reasonably be
expected to frighten, abuse, torment or harass another or in such a way as to
unreasonably interfere with use by others, is prohibited.
The use of CfL Payphones' services without payment for service or attempting to
avoid payment for service by fraudulent means or devices, schemes, false or
invalid numbers, false calling or credit cards or false numbers of such cards, is

prohibited.
CfL Payphones does not transmit messages pursuant to this tariff, but its services
may be used for that purpose.
CfL Payphones' services may be denied for nonpayment of charges or for other
violations of this tariff.
CfL shall not be liable for loss or damage sustained by reason of any failure in or
breakdown of facilities associated with CfL or for any interruption or delay of
services, whatever shall be the cause of such failure, breakdown or interruption
and whether negligent or otherwise, and however long it shall last. In no event
shall CfL's liability for any service exceed the charges applicable under this tariff
to such service.
CfL shall be indemnified and saved harmless by any customer or by any other
entity against claims for libel, slander or the infringement of copyright arising
from the material transmitted over its services; and against all other claims arising
out of any act or omission of a customer or of any other entity in connection with
the services provided by CfL.
CfL Payphones is not liable for any act or omission of any entity furnishing
facilities or services connected with or provided in conjunction with the services
CfL.
CfL Payphones shall not be liable for any personal injury, or death of any person
or persons, nor for any loss or damage sustained by reason of acts, mistakes
omissions, errors or defects in providing its services, whatever shall be the cause
and whether negligent or otherwise.
CfL Payphones shall not be liable for and shall be indemnified and saved harmless
by any customer or other entity from any and all loss, claims, demands, suits
other action or any liability whatever, whether suffered, made, instituted
asserted by any customer or any other entity or any personal injury to or death
any person or persons, and for any loss, damage, defacement or destruction of the
premises of any customer or any other entity of any other property whether owned
or controlled by a customer or any others, caused or claimed to have been caused
directly or indirectly, by any act or omission of a customer or any others or
any installation, operation, failure to operate, maintenance, removal, presence
condition, location or use of services, facilities or equipment provided by CfL
Payphones which is not the direct result of CfL Payphones' negligence. No agents
or employees of any other entity shall be deemed to be the agents or employees of

CfL Payphones.
11.CfL Payphones shall not be liable for any failure of performance due to causes
beyond its control, including, without being limited to, acts of God, fifes, floods
other catastrophes, national emergencies, insurrections, riots or wars, strikes
lockouts, work stoppage or other labor difficulties, acts or omissions of other
carriers, and any law, order, regulation or other action of any governing authority
or agency thereof.
12. The Customer is responsible for payment for use of CfL Payphones services, and
for complying with tariff regulations.
13.The Subscriber and/or Customer must pay CfL Payhones for replacement or repair
of damage to the equipment or facilities of CfL Payphones caused by negligence
or willful act of the Subscriber and/or Customer or others, by improper use of the
services, or by use of equipment provided, by the Subscriber and/or Customer
others.
14. The Subscriber and/or Customer is responsible for compliance with the applicable
regulations set forth in this tariff.
8.15. The Customer is responsible for establishing its identity as often as necessary
during the course of a call.
16. The Customer is responsible for identifying the station, party, or person with
whom communication is desired and/or made at the called number.
The included tariff (price sheet) language does not constitute a determination by the
Commission that a limitation liability imposed by CfL Payphones should be upheld in a
court of law. Acceptance for filing by the Commission recognizes that it is a court'
responsibility to adjudicate negligence and consequential damage claims. It is also the
court's responsibility to determine the validity ofthe exculpatory clause.
