NOTICE OF APPLICATION
YOU ARE HEREBY NOTIFIED that on January 31 , 2006, Cox Idaho Telcom, LLC
Cox ) filed an Application for a Certificate of Public Convenience and Necessity with the
Commission. In the Application, Cox proposes to provide facilities-based basic local exchange
service in the Idaho communities of Ketchum, Sun Valley, Hailey and Bellevue.
YOU ARE FURTHER NOTIFIED that Cox is a Delaware limited liability company
with a principal place of business in Ketchum, Idaho. The Company has a Certificate of Good
Standing issued by the Delaware Secretary of State on December 3 , 2004, a Certificate of
Registration as a Foreign Limited Liability Company issued by the Idaho Secretary of State on
January 24 2006 and a Certificate of Existence issued by the Idaho Secretary of State on January
, 2006.The Company has provided financial information and a letter of financial
commitment, as well as the appropriate contact information for its registered agent for service of
process within Idaho.
NOTICE OF MODIFIED PROCEDURE
YOU ARE FURTHER NOTIFIED that the Commission has reviewed the
Application in this case and has preliminarily determined that the public interest in this matter
may not require a hearing to consider the issues presented, and that the issues raised by the
Application may be processed under Modified Procedure, i., by written submission rather
than by hearing. IDAPA 31.01.01.210-204.
YOU ARE FURTHER NOTIFIED that the Commission may not hold a hearing in
this proceeding unless it receives written protests or comments opposing the use of Modified
Procedure and stating why Modified Procedure should not be used. IDAP A 31.01.01.203.

YOU ARE FURTHER NOTIFIED that the deadline for filing written comments or
protests with respect to Cox s Application and the use of Modified Procedure in this case is
twenty-one (21) days from the service date of this Order. The comment must contain a statement
of reasons supporting the comment. Persons desiring a hearing must specifically request a
hearing in their written comments. Written comments concerning this Application shall be
mailed to the Commission and the Applicant at the address reflected below:

deadline, the Commission will consider them and in its discretion may set the matter for hearing
or may decide the matter and issue its Order based on the written comments before it. IDAP

YOU ARE FURTHER NOTIFIED that persons desiring to intervene in this case for
the purpose of presenting evidence or cross-examining witnesses must file a Petition to Intervene
with the Commission pursuant to Rule 72 and 73 of the Commission s Rules of Procedure
IDAP A 31.01.01.000 et seq. Persons desiring to present their views without parties' rights of
participation and cross-examination are not required to intervene and may present their
comments without prior notification to the Commission or to other parties.
YOU ARE FURTHER NOTIFIED that the Commission has jurisdiction over this
matter pursuant to Title 61 of the Idaho Code.
ORDER
IT IS HEREBY ORDERED that the Application of Cox Idaho Telcom, LLC be
processed by Modified Procedure, IDAP A 31.01.01.201-204. Persons interested in submitting
written comments in this matter must do so no later than twenty-one (21) days from the service
date ofthis Order.
