Pursuant to Idaho Administrative Code IDAPA 31.01.01 Rule 145, Columbine Telephone
Company, Inc. hereby requests that the Commission issue an Order approving this Guaranty on or
before December 27, 2021.
Attached herewith please find the following documents for filing:
- Application of Columbine Telephone Company;
- Form of Loan Agreement stamped confidential;
- Form of Guaranty stamped confidential; and
- Certified copy of Resolutions Providing an Unsecured Guaranty to Horizon
