Columbine Telephone Company, lnc. (Columbine or the Company) hereby petitions the
Commission for an Order approving this Petition for Waiver of the FCC Threshold Numbering
Resource Requirements for Driggs ldaho. ln support hereof, Columbine states as follows:
Columbine is a Title 61 regulated telephone corporation organized under the laws of
the State of Wyoming and is authorized to do business in ldaho and to provide basic and local
exchange telecommunications services pursuant to a Certificate of Public Convenience and
Necessity issued to it by the Commission.
The Company is replacing its existing DMS-10 switch equipment/facilities in its Driggs,
ldaho exchange, and a new NPA-NXX and LRN (local routing number) are required to establish
homing arrangements in LERG 7 SHA in order that new trunking may be ordered and
installed. Once the new switch is operational, existing Columbine customers will be ported to it
from the old switch
On May 9,2OL9, Columbine applied to the North American Numbering Plan
Administrator (NANPA)via the Pooling Administration System (PAS), for a local routing number
(LRN) for the new Driggs switch within its local access and transport area (LATA). On May 9,

20L9, NANPA denied the Company's application because numbering resources already exist for
Company in the LATA and the Thousand-Block Number Pooling Administration Guidelines
preclude NANPA from assigning a service provider a new NXX to establish an LRN unless carrier
has exhausted the assigned resources (see, Attachment A, PAS Part 1 Denial).
The Company's Petition appeals NANPA's denial of its application for additional
numbering resources. Columbine respectfully requests the Commission approve its petition and
direct Commission staffto notify NANPA of its decision and to authorize the release an
NPA/NXX block to the Company for the purpose of an initial LRN; any unneeded 1K blocks will
be returned to NANPA.
WHEREFORE, Columbine Telephone Company, lnc. respectfully requests an expedited
Order of this Commission granting its request and granting such further relief as the
Commission deems just, reasonable and necessary.
