On April 24, 2003 , CenturyTel of the Gem State, Inc. requested to amend Certificate
of Public Convenience and Necessity No. 227 (Second Amended) to reflect that Gem State
Utilities Corporation has changed its name to CenturyTel of the Gem State, Inc.
IT IS HEREBY CERTIFIED that this Third Amended Certificate of Public
Convenience and Necessity No. 227 is issued to CenturyTel of the Gem State, Inc., its successors
and assigns, to own, hold, construct or otherwise acquire, maintain and operate facilities and
supply telephone service to the residents of: Richfield, Grasmere-Riddle, Bruneau, and
Grandview, Idaho, as depicted in the maps provided by CenturyTel (see original file for maps).
The Commission approved the name change from Gem State Utilities Corporation to CenturyTel
of the Gem State, Inc. by Minute Order issued April 25 , 2003.
