On August 23,2011,CenturyTel of the Gem State,Inc.notified the Commission of a
change in its operating entity that affects its Certificate of Public Convenience and Necessity No.227.
On July 28,2009,CenturyTel of the Gem State filed a Certificate of Assumed Business Name with the
Idaho Secretary of State amending the entity name to CenturyTel of the Gem State,Inc.dba
CenturyLink.
IT IS HEREBY CERTIFIED that this Fourth Amended Certificate of Public Convenience and
Necessity No.227 is issued to CenturyTel of the Gem State,Inc.dba CenturyLink,its successors and
assigns,to own,hold,construct or otherwise acquire,maintain and operate facilities and supply telephone
service to the residents of:Richfield,Grasmere-Riddle,Bruneau,and Grandview,Idaho.The Commission
approved this name change to Certificate No.227 by Order No.32344 issued in Case No.CGS-T-l 1-03.
