THIS IS AN IMPORTANT NOTICE REGARDING CHANGES TO YOUR CENTURYLINK VOICE SERVICES We’re sending this notification to let you know that we are making changes that affect your CenturyLink voice services effective May 15, 2020. If you do not agree to the changes to tariffs, catalogs, schedules, and other terms and conditions, then you must take the steps discussed below by May 14, 2020. Based on applicable tariffs, catalogs, schedules and other terms and conditions, we want to remind you that you’re billed in advance for recurring monthly charges associated with your voice services, including charges for features including but not limited to caller ID and voicemail (“Voice Services”), and the applicable taxes and fees for these items. What will change? As of May 15, 2020, or as soon after that date as authorized by state regulatory commissions, your Voice Services and applicable taxes and fees will not be prorated if you cancel these services on any day other than the last day of your billing cycle. These services will continue to be available to you until the end of your billing cycle and you will not receive a refund (pro-rated credits) for any days remaining in your billing cycle after you cancel Voice Services. If you do not agree to be bound by the updated tariffs, catalogs, and rate schedules, please call CenturyLink at 1-855-538-9451 by May 14, 2020 to discuss options, which may include cancellation of your Voice Services. If you instead continue to use your Voice Services, that will constitute your acceptance of the updated tariffs, catalogs, and rate schedules. Customer Requested Termination of Service When business service is terminated at the customer’s request after the minimum service period or initial contract period has been met, service will be discontinued immediately, and a final bill will be issued in accordance with [refer to prorating of bills section]. Residential customers may request termination of residence service at any time, and service will be discontinued on the last day of the customer’s billing cycle after the minimum service period has been met. Final charges will be rendered in accordance with Terms and Conditions, Rendition and Payment of Bills, E. Payment of Bills 1. Payment of bills for telephone service shall be made at the office of the Company or to a duly authorized collector of the Company. 2. Closing bills, special bills, bills rendered on vacation of premises or bills rendered to persons discontinuing exchange service are payable upon presentation. Service Connection Charges and deposits for the establishment or re-establishment of service are payable before service is installed or restored. 3. Restoration of service charges and deposits for the establishment or re-establishment of credit when applicable are payable before service will be established or restored. 4. The Company may require the payment of service connection charges before service is established. 5. Retroactive billing adjustments will not be made for a period exceeding five years. E. Prorating of Opening and Closing Bills Bills for telephone service are normally rendered on a monthly basis. Bills rendered for establishment of services and final bills rendered for discontinuation of business services will be prorated on the basis of a thirty (30) day billing period. Final bills rendered after customer-requested discontinuance of residential service will not be pro-rated and service will remain available to the customer until the first day of the customer’s next billing cycle. Regulations 1. Home Phone II customers may terminate their enrollment in the plan at any time upon notice to the Company, subject to the conditions specified in Terms and Conditions, Rendition and Payment of Bills, E. 2. Unless terminated by the Home Phone II customer or the Company, a customer will remain enrolled in the plan, as amended from time-to-time, with any applicable changes in rate, for as long as the plan continues to be offered by the Company. 3. Home Phone II customers are not eligible for promotional offerings associated with the features included in the plan, unless specifically provided for in a promotional offering. 4. Components of Home Phone II will be converted to ala carte rates for the remaining services if customers remove any of the qualifying services. 5. All terms and conditions specified elsewhere for the respective services/features requested as part of this plan shall apply. 6. Services selected as part of this plan can only be provided where technically available and compatible with other services the customer may choose to order. 7. Home Phone II cannot be combined with any other discounts unless otherwise specified. [1] Effective June 11, 2017, Home Phone II is grandfathered for residential customers.
