On August 23,2011,CenturyTel of the Gem State,Inc.notified the Commission of a
change in its operating entity that affects its Certificate of Public Convenience and Necessity No.
227.On July 28,2009,CenturyTel of Idaho filed a Certificate of Assumed Business Name with
the Idaho Secretary of State amending the entity name to CenturyTel of the Gem State,Inc.dba
CenturyLink.At the August 29,2011 decision meeting,Staff recommended the Commission
change the name on Certificate No.227 to reflect the proper name of the entity conducting
business under authority of the Certificate.The Company has submitted new title pages
reflecting the new assumed business name.
Based on our review of the information provided to the Commission by CenturyTel,
the Commission finds it reasonable to issue an amended Certificate of Public Convenience and
Necessity to reflect the change in name on Certificate No.227 from CenturyTel of the Gem
State,Inc.to CenturyTel of the Gem State,Inc.dba CenturyLink.
ORDER
IT IS HEREBY ORDERED that Certificate of Public Convenience and Necessity No.
227,is amended to change the name from CenturyTel of the Gem State,Inc.to CenturyTel of the
Gem State,Inc.dba CenturyLink.CenturyTel will be issued an amended Certificate reflecting
this name change.
THIS IS A FINAL ORDER.Any person interested in this Order may petition for
reconsideration within twenty-one (21)days of the service date of this Order.Within seven (7)
days after any person has petitioned for reconsideration,any other person may cross-petition for
reconsideration.See Idaho Code §6 1-626.
