On April 7, 1998, CenturyTel of Idaho, Inc. requested to amend Certificate of Public
Convenience and Necessity No. 289 (Second Amended) to reflect that Century Telephone of
Idaho, Inc. has changed its name to CenturyTel of Idaho, Inc.
lT IS HEREBY CERTIFIED that this Third Amended Certificate of Public
Convenience and Necessity No. 289 is issued to CenturyTel of Idaho, Inc., its successors and
assigns, to own, hold, construct or otherwise acquire, maintain and operate facilities and supply
telephone service to the residents of: Salmon, Leadore and North Folk, Idaho. The Commission
approved the name change from Century Telephone of Idaho, Inc. to CenturyTel of Idaho, Inc.
by Minute Order issued July 22 2003.
