CenturyTel of Idaho, Inc., dba CenturyLink (CenturyLink), by and through its
undersigned attorneys, hereby files its Notice of Election in accordance with the
provisions of Idaho Code § 62-605. By this Notice, CenturyLink elects to make all of its
retail services offered in the service territory of CenturyTel of Idaho, Inc. subject to the
provisions of Title 62, Idaho Code.
CenturyTel of Idaho's Notice of Election - 1 -
I. NOTICE OF ELECTION
A. Idaho Code § 62-605 Requirements
1.Subsection (1) (a): Name and address of the telephone corporation.
CentiiryTel of Idaho, Inc., dba CenturyLink is a Delaware corporation and is
authorized to do business in Idaho. CenturyLink's business address in Idaho is 999 Main
Street, Boise, Idaho, 83702. Correspondence should be directed as follows:

2.Subsection (1) (b): Narrative description of telecommunications services
provided and geographic area served
CenturyLink in its CenturyTel of Idaho operations offers an array of intra- and
interstate telecommunications services to individuals and businesses in the exchanges of
Salmon, North Fork and Leadore, Idaho. All of these services are described in the
following tariffs, which are on file with this Commission:
CentuiyTel of Idaho, Inc. /dba CenturyLink - General Exchange Tariff
CentuiyTel of Idaho, Inc. /dba CenturyLink - Access Service Tariff
CenturyTel of Idaho, Inc. /dba CenturyLink - Wholesale Services Tariff
For notice purposes, these tariffs are incorporated herein by reference.
CenturyTel of Idaho's existing service area is more particularly defined in the exchange
maps on file with the Commission and in the territorial descriptions set forth as part of
CenturyTel of Idaho's Notice of Election -2-
the Company's existing Certificate of Public Convenience and Necessity No. 289, issued
January 14, 1982, as amended. For notice purposes these exchange maps and the
certificate are incorporated herein by reference.
B.Services Subject to this Notice of Election
All of the services offered by CenturyLink in the Centurylel of Idaho tariffs
listed above are the subject of this Notice of Election.
C.Services CenturyLmk Intends to Leave Subject to the Provisions of Title 61,
Idaho Code
CenturyLink intends that all of its services be offered under Title 62, Idaho Code
upon the effective date of this Notice.
D.CenturyLmk Catalogs
CenturyLink will file the following Catalogs with this Commission on or before
May 31, 2013. These Catalog filings will be incorporated in this Notice as follows:
Attachment A provides a copy of the CenturyTel of Idaho, Inc. /dba CenturyLink -
General Exchange Catalog (Catalog No. 1), which will replace the General Exchange
Tariff on the effective date of this Election. Attachment A contains no substantive
changes to the prices or terms and conditions of the products and services offered; it
merely reflects the changes necessary to make the document consistent with regulation
under Title 62, Idaho Code together with changes reflecting CenturyTel of Idaho's intent
to do business using the CenturyLink name.
Attachment B provides a copy of the CenturyTel of Idaho, Inc. /dba CenturyLink
Access Service Catalog (Catalog No. 3) which will replace the Access Service Tariff on
the effective date of this Election. This Catalog has been drafted to reflect the access rate
changes that the Company must make to comply with the LISP/ICC Transformation
CenturyTel of Idaho's Notice of Election -3-
Order.' Attachment B also reflects CenturyTel of Idaho's intent to do business using the
CenturyLink name.
Attachment C provides a copy of the Centurylel of Idaho, Inc. /dba CenturyLink
- Wholesale Services Catalog (Catalog No. 5). Attachment C also reflects CenturyTel of
Idaho's intent to do business using the CentuiyLink name but otherwise contains the
same services and prices contained in the present Wholesale Catalog. The effective date
of these Catalogs is July 2, 2013. On that date CenturyLink's tariffs will be withdrawn in
their entirety.
Attachments A-C reflect the availability, price, and terms and conditions for the
services CenturyLink will provide pursuant to Title 62, Idaho Code. All of the services
that are the subject of the Notice, with the exception of certain of its access charges
which are being changed to comply with federal regulation, are offered in the above-
described Catalogs at prices identical to those contained in the respective CentuiyTel of
Idaho tariffs for the same services. After the effective date of the Catalogs, any future
price changes will be governed by Title 62, Idaho Code.
E. Customer Notice
CenturyTel of Idaho provided the following notice to all of its customers in the
May billing cycle:
Effective July 2, 2013, the regulation of CenturyLink by the Idaho Public
Utilities Commission will be relaxed pursuant to Title 62, Idaho Code.
While Title 62 allows CenturyLink to change services prices without
