Application is hereby made to the Idao Public Utilities Commssion for a Cerficate of Public
Convenience and Necessity authorizing BLC Management LLC d//a Angles Communcation
Solutions, ("Applicant" or "BLC") to provide local exchange telecommuncations services pursuant to
Idaho Code Sections 61-526 through -528 and IDAPA 31.01.01.111 (Riles 111 & 112) as clarfied by
Proceural Order No. 26665 in Case No. GNR-T-96-4. The following general information and
exhbits are fushed in support thereof:

The Applicat has no offce located in the State of Idaho. The Applicant intends to provide
resold and facilities-based local exchange serce utilizing unbundled network elements, or the
equivalent thereof. Such services wil be provided by utilizing the facilities incumbent local
exchange carrers (ltLECslt).
2.) Applicant is incorporated in the State of Tennessee and is in good stading under the laws of
that state. In addition, the Company is authorized to do business as a foreign BLC in the State of
Idaho. Attched as Exhbit 1 to ths Application is a copy of the Company's Aricles of Incorporation.
A copy of Applicant's certficate of authority to tranact business in Idaho is attached hereto as
Exhbit 2.

8.) Applicant intially proposes to provide resold local exchange serices and to purchase
unbundled network elements provided by existig LECs: Qwest Nort, Qwest South, and Veron.
Applicant has no curent plans to instal facilities in Idaho but may do so in the futue, however, the
natue and extent of the facilities to be utilized has yet to be determned. Applicant intends to provide
all forms of intrastate local exchange and interexchange telecommuncations serces including:

1. Interexchange (switched and dedicated serices):
A. 1 + and 10 1XX outbound dialing;
B. 800/888 toll-free inbound dialing;
C. Prepaid and Postpaid callig cards;
D. Directory Assistance; and
E. Frame Relay and other data serces.
2. Loca Exchange:
A. Loca Exchange Serces for business and residence customers that will enable
customers to origiate and termate loca cals in the local calling area sered by other
LECs, including local dial tone and custom calling featues.
B. Switched local exchange serces such as flat-rated and mease-rated local serces;
vertica servces, Direct Inward and Outward Dialed trs, caer access, public and
sem-public coin telephone serces, and any other switched local serces that
curently exist or wil exist in the futue.
C. Non-switched local serces (e.g., private line) that curently exist or wil exist in the
futue.
D. Centrex and/or Centrex-like servces that curently exist or wil exist in the futue.
E. Digita subscriber line, ISDN, and other high capacity line serces.
Applicant seeks authority to resell and provide facilities-based interexchange and local
exchange serices intially thoughout the State where provided by incumbent LECs, however,
Applicant does not intend to serice areas serce by any LECs which are eligible for a small or rual
carer exemption puruat to Section 251 of the Federal Telecm Act of 1996.

Applicant has no curent plans to intal facilities in Idaho. IfBLC intals facilities in Idaho, it
would probably use the following or a simlar confguation of equipment: BLC wil provide voice
and high speed data serces though a combination of the latest technology switchig and transport
media comprised of the Lucent Technology 5 ESS Generic 13 switch module, ADSUSDSL
transport and Internet serice equipment and the latest Optical multiplexer DAC's confguations.
The switching system consists of a central processing and control complex capable of
interconnection as a peer to the incumbent as well as competitive local exchange companes. The
hub portion of the switch wil interconnect with the public switched network on Signaling System 7
(ltSS7") or Feature Group D ("FGDIt) facilities. The system's remote module capability will allow
properties to be served in a maner that provides the exchange of appropriate signaling, control and
calling/caller information to the network in accordance with network standards and specifications.
Additionally, these services wil be delivered over a combination of deliver mechanisms though
incumbent local carrers' unbundled loop network, both copper and fiber and transport networks, as
well as via BLC constrcted facilities. Applicant intends to provide servce upon cefication and
fialization of interconnection agreements with the LECs.

Balance Sheet as of December 31, 2007, Exhbit 3, are being filed in a separately sealed envelope
marked "Confidential and Proprietar.

proposed taff establishig its proposed serces and charges upon completion of interconnection and
upon receipt of cerfication by the Commssion.

telecommuncations services in Idaho and will provide serice in accrdace with all laws, rules and
regulations to the extent they are not preempted by the Federal Act.
15.) Applicant will not require advance payments or deposits, therefore, no escrow accunt is being
filed.

the Idaho Public Utilities Commssion enter an order granting a Certficate of Public Convenience and
Necessity authorizing BLC Management LLC d//a Angles Communcation Solutions, to provide
resold and facilities-based local exchange telecommuncations servces pursuat to Idaho Code

SEClION 1.0 - DEFINITIONS
For the purpose of th taif, the followig deftions wi apply:
Access Line - An arangement which connects the Customer's location to a carner's switcg center or
point of presence.
Account Codes - Optiona, Customer-defied digits that alow the Customer to identi the individual user,
department or client associated with a cal. Account Codes appear on the Customer bil.
Advance Payment - Part or al of a payment requied before the sta of servce.
Authorized User - A peson, fim, corporation, or any other entity authorid by the Customer to
communcate utiing the Company's servce.
Business - A clas of service provided to individual engaged in business, fi, parterships, corporations,
agencies, shops, works, tenats of offce buidigs, and individual practicig a profession or operatig a
business who have no offces other than their residences and where the use of the servèe is priary or
substatialy of a business, professional or occupational natue.
Commssion - Idaho Public Utities Commsion.
Company or Carer - BLC Mangement LLC d/b/a Angles Communcation Solutions, uness otherwe

Customer - The person, fi, corporation or other entity which orders, cancels, amends or uses servce and is

extension diectly without the intervention of the Company operator.
Dial Pulse (or "DP") - The pule tye employed by rota dial station sets.
Dual Tone Multi-Frequency (or "DTMF") - The pule ty employed by tone dial station sets.
End User - Any person, fim, corporation, parership or other entity which uses the servces of the
Company under the proviions and regutions of th ta. The End User is responsible for payment
uness the charges for the servces utid are accepted and paid for by another Customer.
End Ofice - With respect to each NP A-NX code prefi assigned to the Company, the location of the
Company's "end offce" for puroses of th taf shal be the point of interconnection associted with tht
NP A-NX code in the Local Exchage Routig Guide ("LERG"), issued by Bellcore.
Hearg Impaied - Those persons with communcation impaients, includig those hearing impaied,
deaf, deaf/blid, and speeh impaied persons who have an impaient that prevents them from
communcatig over the telephone without the aid of a telecommuncations device for the deaf.
Hunting - Route a cal to an idle station lie in a prearranged group when the caled station lie is bus.
In-Only - A servce attbute that restrct outward dial access and route incomig cal to a designted
anwer point.
IXC or Interexchange Caer - A long ditace telecommuncations servces provider.

Mium Point of Presence ("MPOP") - The mai telephone closet in the Customer's buidig.
Monthy Recg Charges - The monthy chages to the Customer for servces, facilties and equipment,
which contiue for the agreed upon duration of the servce.
Multi-Frequency or ("MF") - An inter-mache pule tye used for signalg betwee telephone switches, or
between telephone switches and PBX/key systems.
Non-Recurg Charge ("NRC") - The inti charge, usualy assessed on a one-tie basis, to intite and
establih service.
Other Telephone Company - An Exchange Telephone Company, other than the Company.
PBX - Private Branch Exchange
Premises - A buidig or buidigs on contiguous proper.
Recurg Charges - The monthy charges to the Customer for servces, facilties and equipment which
contiue for the agreed upon duration of the service.
Residence or Residential - A clas of servce fuhed to a Customer at a place of dwellg where the actal
or obvious use is for domestic puroses.

Serce Commencement Date - The fist day followig the date on which the Company noties the
Customer that the requested servce is avaiable for use, uness extended by the Customer's refual to accept
service which does not conform to stadards set forth in the Servce Order and th taif, in which cas the
Servce Commencement Date is the date of the Customer's acceptace. The Company and Customer may
mutualy agree on a substitute Servce Commencement Date.
Servce Order - The wrttn request for services execte by the Customer and the Company in the format
devied by the Company. The signg of an Order by the Customer and acceptace by the Company
intites the respective obligations of the pares as set fort therein and pursuant to th ta, but the
duration of the service is calcuted from the Service Commencement Date.
Telecommunications Company or Provider - Used thoughout th ta to mean BLC Management LLC

SECTON 2.0 - RULES AN REGULATIONS
2.1 Undertakng of the Company
2.1.1 Scope
The Company undertaes to fuh communcations service puruant to the term of th
ta in connection with one-way and/or two-way inormtion tranmision origitig
from points with the State of Idaho, and termtig with a local calg area as defied
herein.
The Company is responsible under th ta only for the services and facilties provided
hereunder, and it assumes no responsibilty for any service provided by any other entity
tht purchases access to the Company network in order to origiate or termate its own
services, or to communcate with its own Customers.
2.1.2 Shortage of Equipment or Facilities
(A) The Company reserves the right to lit or to alocate the use of exitig facities,
or of additiona facities offered by the Company, when necessar beause of lack
of facilties, or due to some other cause beyond the Company's control.
(B) The fuhig of service under th ta is subject to the availabilty on a
contiuig basis of al the necessar facities and is lited to the capacity of the
Company's facilties as well as facities the Company may obta from other
cariers to fuh service from tie to tie as requied at the sole dietion of the
Company.

2.1.3 Terms and Conditions
(A) Service is provided on the basis of a mium period of at least one month, 24
hours per day. For the purpose of computig chges in th ta, a month is
considered to have thty (30) days.
(B) Customers may be requied to enter into wrtten service orders which shal conta
or reference a specifc description of the service ordered, the rates to be charged, the
duration of the services, and the term and conditions in th tarif. Customers wi
alo be requied to execute any other docents as may be reasonably requested
by the Company.
(C) Except as otherwe stated in the ta, at the expiration of theintial term specifed
in each Servce Order, or in any extension thereof, servce shal contiue on a month
to month basis at the then cuent rates uness termited by either par upon
proper notice. Any termation shal not relieve the Customer of its obligation to
pay any charges incurred under the serce order and th ta prior to
termation. The rights and obligations which by their natue extend beyond the
termtion of the term of the service order shal surive such termtion.
(D) Service may be termated upon wrttn notice to the Customer if:
(1) the Customer is using the service in violation of th ta; or
(2) the Customer is using the servce in violation of the law.
(E) Th ta shal be interpreted and governed by the laws of the State of Idaho
without regard for its choice of laws provision.

(H) The Company hereby reserves its rights to establih servce packages specifc to a
particula Customer. These contracts mayor may not be associated with volume
and/ or term discounts.

2.1.4 Limitations on Liabilty
(A) The liabilty of the Company for damages ariing out of the furnhig of its
Servce, including but not lited to mitaes, omisions, interruption, delay, or
errors, or other defects, representations, or use of these services or damages
arising out of the failure to fuh the service, whether caused by acts or
omision, shal be lited to the extension of alowances for interruption as set
forth in 2.7. The extension of such alowances for interruption shal be the sole
remedy of the Customer and the sole liabilty of the Company. The Company
wi not be lible for any direct, indiect, incidental, special, consequential, lost
profits, exemplar or punitive damages to Customer as a result of any Company
service, equipment or facilties, or the acts or omisions or negligence of the
Company's employees or agents.
(B) The Company's liabilty for wil miconduct, if establihed as a result of
judicial or admintrative proceedigs, is not lited by th tarif. The
Company's liabilty, if any, with regard to delayed intallation of Company
facilties or commencement of servce, shall not exceed $1,000. With respect to
any other clai or suit, by a Customer or by any others, for damage associated
with the ordering (including the reservation of any specifc number for use with a
service), intallation (includig delays thereof), provision, termiation,
maitenance, repair, interruption of restoration of any service or facilties offered
under th taif, and subject to the provisions of Section 2.7, the Company's
liabilty, if any, shal be liited as provided herein.
(C) The Company shall not be liable for any delay or faiure of performance or
equipment due to causes beyond its control, includig but not lited to: acts of
God, fie, flood, explosion or other catastrophes; and law, order, regulation,
diection, action or request of the United States governent or of any other
governent, including state and local governments having or claimg
jurisdiction over the Company, or of any department, agency, commsion,
bureau, corporation or other intrentality of anyone or more of these federal,
state, or local goverIÙents, or of any mitary authority; preemption of exitig
service in compliance with national emergencies; inurrections; riots; wars;
unvailabilty of rights-of-way or material, or strikes, lockouts, work stoppages,
or other labor difficulties.

(D) The Company shal not be lible for (a) any act or omision of any entity
fuhig the Company or the Company's Customers facilties or equipment
used for or with the services the Company offers; or (b) for the acts or omisions
of common carriers or warehousemen. Except in 31.41.01 Rule 501, 502, and 503
of the IDAP A.
(E) The Company shal not be lible for any damages or losses due to the fault of
negligence of the Customer or due to the faiure or malfctiori of Customer-
provided equipment or facilties.
(F) The Customer shal indemn and hold the Company harmless from any and al
loss, claim, demands, suits, or other action, or any liabilty whatsoever, whether
sufered, made, intituted, or asserted by any other party or person(s), and for
any loss, damage, or destrction of any property whether owned by the
Customer or others, caused or claimed to have been caused directly or indirectly
by the intallation, operation; faiure to operate, maitenance, removal, condition,
location, or use of intallation provided by the Company. The Company reserves
the right to require each Customer to sign an agreement acknowledgig
acceptace of the provisions of th section as a condition precedent to such
intalltions.
(G) The Company shal not be lible for any defacement of or damage to Customer
premis resultig from the furnhig of services or equipment on such premies
or the intallation or removal thereof, uness such defacement or damage is
caused by gross negligence or wil miconduct of the Company's agents or
employees. No agents or employees of other participatig carriers shall be
deemed to be agents or employees of the Company.
(II Notwthtanding the Customer's obligations, the Company shall be indemnied,
defended, and held harmless by the Customer or by others authorized by it to
use the servce againt any claim, loss of damage arising from Customer's use of
services funihed under this tariff, includig: clai for libel, slander, invasion
of privacy or iningement of copyrght arising from the material, data,
inormation, or other content tranmittd via the Company's service; and patent
iningement claim arising from combing or connectig the service offered by
the Company with apparatus and systems of the Customer or others. All other
clai arising out of any act or omision of the Customer or others, in connection
with any service provided by the Company pursuant to th tarif.

(I) The entie liabilty of the Company for any claim, loss, damage or expense from
any cause whatsoever shal in no event exceed sum actually paid to the
Company by Customer for the specifc services giving rise to the claim, and no
such action or precedig agait the Company shal be commenced more than
one year afr the service is rendered.
(J The Company makes no warranties or representations, express or implied,
including warranties of merchantabilty or fitness for a particulr use, except
those expressly set forth herein.
(K) The Company shall not be liable for any act or omission of any other company or
companes furnihig a porton of the service, or for damages associated with
service, chanels, or equipment which it does not furnih, or for damages which
result from the operation of Customer-provided systems, equipment, facilties or
services which are interconnected with Company services. Except in 31.41.01
Rule 501, 502, and 503 of the IDAP A.
(L) The Company does not guarantee nor make any waranty with respect to servce
intaations at locations at which there is present an atmosphere that is
explosive, prone to fie, dangerous or otherwie unuitable for such intallations.
The Customer shal indemn and hold the Company harmless from any and all
loss, claim, demands, suits or other action, or any liabilty whatsoever, whether
suffered, made, intituted or asserted by the Customer or by any other part, for
any personal injur to, or death of, any person or persons, or for any loss,
damage or destrction of any property, whether owned by the Customer or
others, caused or claied to have been caused, diectly or indirectly, by the
intalltion, operation, faiure to operate, maitenance, removal, presence,
condition, locations or use of service furnhed by the Company at such locations.

(M With respect to Emergency Number 911 Servce:
(a) Th service is offered solely as an aid in handing assistace cal inconnection with fie, police and other emergencies. The Company is not
responsible for any losses, clai, demands, suits or any liabilty whatsoever,
whether suffered, made, intituted or asserted by the Customer or by any
other party or person for any personal injur to or death of any person or
persons, and for any loss, damage or deshuction of any propert, whether
owned by the Customer or others, caused or claied to have been caused by:
(1) mitakes, omisions, interruptions, delays, errors or other defects in theprovision of th service, or (2) intallation, operation, faiure to operate,
maintenance, removal, presence, condition, location or use of any equipment
and facilties fuhig th service.
(b) The Company is not responsible for any inringement or invasion of the rightof privacy of any person or persons, caused or claied to have been caused,
directly or indirectly, by the intallation, operation, failure to operate,
maintenance, removal, presence, condition, occasion or use of emergency 911
service features and the equipment associated therewith, or by any services
funihed by the Company including, but not lited to, the identication of
the telephone number, address or nae associated with the telephone used
by the party or parties accessing emergency 911 service, and which arise out
of the negligence or other wrongf act of the Company, the Customer, its
users, agencies or muncipalties, or the employees or agents of anyone of
them.
(N) The Company's libilty arising from errors or omisions in Directory Listigs,
other than charged litig, shall be lited to the amount of actual impairment of
the Customer's servce and in no event shal exceed one-half the amount of the
fixed monthly charges applicable to exchange service afected during the period
covered by the directory in which the error or omision occurs. . In. cases of
charged Directory Listigs, the liabilty of the Company shall be . lited to an
amount not exceeding the amount of charges for the charged litigs involved
during the period covered by the diectory in which the error or omision occurs.

(0) When a Customer with a non-publihed telephone number, as defied herein,
places a cal to the Emergency 911 Service, the Company wi release the name
and address of the calg party, where such inormation can be determied, to
the appropriate local governental authority responsible for the Emergency 911
Service, upon request of such governmenta authority. By subscribing to service
under th taif Customer acknowledges and agrees with the release of
inormation as described above.
(P) The included taf language does not constitute a determiation by theCommsion that a litation of liabilty imposed by the Company should be
upheld in a court of law. Acceptace for filg by the Commsion recogn
that it is a court s responsibilty to adjudicate negligence and consequenti
damage clai. It is also the court s responsibilty to determe the valdity of
the exculpatory clause.
2.1.5 Notication of Servce-Afecting Activities
The Company wi provide the Customer reasonable notication of service-afectig

Customer to determie the reasonable notication requiements. With some emergency or
unplaned service-afectig conditions, such as an outage resultig from cable damage,
notication to the Customer may not be possible.

2.1.6 Provision of Equipment and Facilties
(A) The Company shal use reasonable effort to mata only the facilties and
equipment that it fuhes to the Customer. The Customer may not nor may the
Customer permit others to rearange, diconnect, remove, attmpt to repai, or
otherwe interfere with any of the facilties or equipment intaed by the
Company, except upon the wrttn consent of the Company.
(B) The Company may substitute, change or rearrange any equipment or facilty at any
tie and from tie to tie, but shal not thereby alter the tehncal parameters of
the service provided by the Customer.
(C) Equipment the Company provides or înta at the Customer Premies for use in
connection with the servces the Company offers shal not be used for any purpose
other than that for which the equipment is provided.
(D) Except as otherwe indicated, Customer provided station equipment at the
Customer's premis for use in connecon with the servce shall be so constrcted,
mataed and operate as to work satifactoriy with the facilties of the
Company.
(E) The Company shal not be responsible for the intation, operation, or
maitenace of any Customer provided communcations equipment. Where such
equipment is connected to the facilties fuhed pursuant to th ta, the
responsibilty of the Company shal be lited to the fuhig of facilties offered
under th ta and to the maiteance and operation of such facilties. Subject to
th responsibilty, the Company shall not be responsible for:
(1) the though tranmision of sign by Customer provided equipment or
for the qualty of, or defects in, such tranmision; or
(2) the reception of signal by Customer-provided equipment; or
(3) network control signg where such signalg is performed by Customer-
provided network control signalg equipment.

2.1.7 Non-Routie Installation
At the Customer's request, intation andl or maintenance may be performed outside the
Company's regu business hours or in hazdous locations. In such cases, charges based
on cost of the actual labor, materiaL or other costs incued by or charged to the Company
wi apply. If intation is stated durg reguar business hour but, at the Customer's
request, extends beyond reguar business hours into tie periods includig, but not lited
to, weekends, holidays, andl or night hours, additiona charges may apply.
2.1.8 Special Constrction
Subject to the agreement of. the Company and to al of the regutions contaed in th
ta, specia constrction or facities may be undertaken on a reasonable eforts basis at the
request of the Customer. Special constrction is constrction undertaen:
(A) where facilties are not presently avaible, and there is no other requirement for the
facilties so constrcted;
(B) of a tye other than that which the Company would normaly utie in thefuhig of its services;
(C) over a route other than that which the Company would normaly uti in thefuhig of its servces;
(D) in a quantity greater than that which the company would normaly constrct;
(E) on an expedited basis;
(F) on a temporar basis unti permanent facities are avaiable;
(G) involvig abnormal costs; or
(H) in advance of its normal constrction.

2.2 Prohibited Uses
2.2.1 The servces the Company offers sha not be used for any unawfu purpose or for any use
as to which the Customer has not obtaed al requied governenta approvals,
authoriations, licenses, consents and permts.
2.2.2 The Company may requie applicants for servce who inted to use the Company's
offerigs for resale and/or for shared use to fie a lettr with the Company confg that
their use of the Company's offerings complies with relevant laws and the Idaho
Commsion's reguations, policies, orders, and decisions.
2.2.3 The Company may block any signal being tranmittd over its Network by Customers
which cause interference to the Company or other users. Customer shal be relieved of al
obligations to make payments for charges relatig to any blocked Service and shal
indemn the Company for any clai, judgment or liabilty resultig from such blockage.
2.2.4 A Customer, joint user, or authorid user may not assign, or tranfer in any maner, the
servce or any rights associted with the servce without the writt consent of the
Company. The Company wi permt a Customer to tranfer its existig servce to another
entity if the exitig Customer has paid al charges owed to the Company for reguated
communcations services. Such a tranfer wi be treated as a diconnection of exitig
servce and intation of new service, and non-recg intation charges as stated in
th tarif wi apply.

equipment, space and power to operate Company facilties and equipment
intaed on the premies of the Customer, and the level of heatig and ai
conditionig necessar to mata the proper operatig environment on such
premies;
(D) obtag, maitag, and otherwe having fu responsibilty for al rights-of-way and conduit necessar for intaation of fiber optic cable and assocte
equipment used to provide Communcations Services to the Customer from the
cable buidig entrance or propert lie to the location of the equipment space
described in Setion 2.3.1(Q. Any and al costs associated with the obtag and
maitag the rights-of-way descrbed herein, includig the costs of altering the
strctue to permt intation of the Company provided facilties, shal be borne
entiely by, or may be charged by the Company, to the Customer. The Company
may requie the Customer to demonstrate its complice with th section prior to
acceptig an order for servce.

(e.g., frble asbetos) prior to any constrction or intaation work;
(F) complyig with al laws and regulations applicable to, and obtag al consents,
approval, licens and permts as may be requied with respet to, the location of
Company facilties and equipment in an Customer premies or the rights-of-way
for which Customer is responsible under Seon 2.3.1(0); and grantig or obtag
permsion for Company agents or employees to enter the premies of the
Customer at any tie for the purose of intag, inpectig, maitag,
repaig, or upon termition of servce as stated herein, removig the facilties or
equipment of the Company;
(G) not creatig or alowig to be placed any liens or other encubrances on the
Company's equipment or facilties; and
(H) makg Company facities and equipment availble periodicaly for maitece
puroses at a tie agreeable to both the Company and the Customer. No
alowance wi be made for the period durg which service is interrpted for such
purposes.

(A) The Customer wi be lible for damages to the facilties of the Company and for al
incidenta and consequenti damages caused by the negligent or intetiona acts or
omisions of the Customer, its offcers, employees, agents, invites, or contractors
where such acts or omisions are not the diect result of the Company's negligence
or intetiona miconduct
(B) To the extent caused by any negligent or intentional act of the Customer as
described in (A), precedig, the Customer shal indemn, defend and hold
harmless the Company from and agait al cla, actions, damges, libilties,
costs and expes, including reasonable attorneys' fees, for (1) any loss,
destrction or damage to property of any thd par, and (2) any libilty incured
by the Company to any thd par pursuat to th or any other taif of the
Company, or otherwise, for any interrption of, interference to, or other defect in
any servce provided by the Company to such thd par.
(C) The Customer shal not assert any clai agait any other Customer or user of the
Company's services for damages resultig in whole or in party from or ariing in
connecon with the fuhig of service under th ta includig but not lited
to mitaes, omisions, interrptions, delays, errors or other defec or
mireprestations, whether or not such other Customer or user contrbuted in any
way to the occuence of the damages, uness such damages were caused solely by
the negligent to intentional act or omision of the other Customer or user and not
by any act or omision of the Company. Nothg in th ta is inteded either to
lit or to expand Customer's right to assert any clai agait thd pares for
damages of any natue other than those descrbed in the precedig setence.

2.4 Customer Equipment and Chanels
2.4.1 General
A user may tranmit or receive inormation or signal via the facities of the Company. The
Company's servces are designted priarily for the tranmision of voice-grade telephonic
sign, except as otherwe stated in th taif. A user may tranmit any form of sign that
is compatible with the Company's equipment, but the Company does not guarante that its
services wi be suitable for puroses other than voice-grade telephonic communcation
except as speifcaly stated in th ta.
2.4.2 Station Equipment
(A) Term equipment of the user's premies and the electrc power consumed by
such equipment shal be provided by and maitaed at the expens of the user.
The user is responsible for the proviion of wig or cable to connec its termi
equipment to the Company MPOP.
(B) The Customer is responsible for ensurg that Customer-provided equipment
connected to Company equipment and facilties is compatible with such equipment
and facilties. The magntude and character of the voltages and cuents impressed
on Company-provided equipment and wig by connection, operation, or
maintenance of such equipment and wig shal be such as not to cause damage to
the Company-provided equipment and wig or inju to the Company's
employees or to other persons. Any additiona proteve equipment requied to
prevent such damge or injur shal be provided by the Company at the
Customer's expense, subject to prior Customer approval of the equipment expense.

2.4.3 Interconnection of Facilties
(A) Local Trafc Exchge provides the abilty for another local exchange provider to termte
local trafc on the Company's network. In order to qual for Loca Trafc Exchage the
cal must (a) be origited by an end user of a company tht is authoriz by the
Commsion to provide local exchange servce; (b) origite and termate with a local
calg area of the Company.
(B) Any speia interface equipment necessar to acheve compatibilty between the facilties
and equipment of the Company used for fuhig Communcations Services and. the
chanels, facilties, or equipment of others shal be provided at the Customer's expene.
(C) Communcations Services may be connected to the servces or facilties of other
communcations carers Oiùy when authorid by, and in accordance with, the term and
conditions of the taifs of the other communcations cariers which are applicable to such
connections.
(D) Facilties fuhed under th ta may be connecte to Customer provided termal
equipment in accordance with the provisions of th ta. Al such termal equipment
shal be regitered by the Federal Communcations Commsion pursuant to Part 68 of Title
47, Code of Federal Regutions; and all user-provided wig shal be intaed and
maintaed in complice with those regutions.

2.4.4 Inspectons
(A) Upon suitable notication to the Customer, and at a reasonable tie, the Company
may make such tests and inpections as may be necessar to determe that the
Customer is complyig with the requiements set forth in Setion 2.4.2(B) for the
intation, operation, and maitenace of Customer-provided facities,
equipment, and wiing in the connection of Customer-provided facilties and
equipment to Company-owned facities and equipment
(B) If the protetive requiements for Customer-provided equipment are not being
complied with, the Company may tae such action as it deems necessar to protet
its facilties, equipment, and personnel. The Company wi noti the Customer
promptly if there is any need for fuer correcve action. With te days of
receiving th notice, the Customer must tae th corrective acton and noti the
Company of the acton taen. If the Customer fai to do th, the Company may
tae whatever additional action is deemed necessar, includig the suspension of
service, to protect its facities, equipment and personnel from har.
(C) If harm to the Company's network, personnel or serces is iment, the
Company reserves the right to shut down Customer's service imediately, with no
prior notice requied.

2.6 Payment Arangements
2.6.1 Payment for Servces
The Customer is responsible for the payment of al charges for facilties and servces
fuhed by the Company to the Customer.
The Customer is responsible for the payment of federal excise taes, state and local sales
and use taes and sim taes imposed by goverenta jurisdictons, al of which shal be
separately designated on the Company's invoices. Any taes imposed by a local jurdicton
(e.g., county and mUlcipal) wi only be recovered from those Customers residig in the
afected judictions.
2.6.2 Billng and Collection of Charges
The Customer is responsible for payment of all charges incued by the Customer or other
usrs for servces and facilties furnhed to the Customer by the Company.
(A) Non-recring charges are due and payable with th (30) days afr the date
the invoice is maied to the Customer by the Company.
(B) The Company shal prest invoices for recurrig charges monthy to the
Customer, in advance of the month in which servce is provided, and recg
charges shal be due and payable with thty (30) days afer the date the invoice is
maed to the Customer by the Company. When bilg is based upon Customer
usage, usage charges wi be biled monthy for the precedig bilg period.
(C) When servce does not begi on the fist day of the bilg period, or end of the lat
day of the bilg period, the charge for the fraction of the month in which servce
was fuhed wi be calcuated on a pro rata basis. For th purpose, every month
is considered to have th (30) days.

(D) Bilg of the Customer by the Company wi begi on the Service Commencement
Date, which is the day on which the Company noties the Customer that the
service or facity is avaible for use, except that the Servce Commencement Date
may be postponed by mutual agreement of the pares, or if the servce or facity
does not conform to stadards set fort in th taif or the Service Order. Bilg
accrues though and includes the day tht the servce, circut, arangement or
component is dicontiued.
(E) If any porton of the payment is not received by the Company with 30 days of
receipt of th bil, or if any portion of the payment is received by the Company in
fuds which are not imediately avaible upon presentment, then a late payment
charge of 1.5% per month shal be due to the Company. A late payment charge is
not applicable to subsequent rebilg of any amount to which a late payment
charge has aleady ben applied. Late payment charges are to be applied without
discriation.
(F) the Customer should noti the Company of any diputed items on an invoice
with sixty (60) days of receipt of the invoice. If the Customer and the Company
are unable to resolve the dipute to their mutual satifaction, the Customer may fie
a complaint with the Commsion in accordance with the Commsion's rules and

The date of the dispute shal be the date the Company receives suficient
documentation to enable it to investigate the dispute. The date of the resolution
is the date the Company completes its investigation and notiies the Customer of

(G) If servce is diconnected by the Company (in accordance with Seon 2.6.3followig) and later re-intaed, re-intation of servce wi be subject to al
applicable intation chges. If servce is suspended by the Company (in
accordance with Setion 2.6.3 followig) and later restored, restoration of servce
wi be subjec to the rate in Setion 4.3.

2.6.3 Discontiuance of Servce for Cause
The Company may dicontiue servce for the followig reasons provided in th Seon
2.6.3. Customers wi be provided ten (10) days writtn notice prior to dicontiuance
uness otherwe indicated. Notice wi bë provided via Firt Oass U.S. Mai.
Upon the Company's diontiuance of servce to the Customer under Setion 2.6.3(A) or
2.6.3(B), the Company, in addition to al other remedies that may be avaible to the
Company at law or in equity or under any other proviion of th ta, may declare al
futue monthy and other charges which would have ben payable by the Customer durg
the remainder of the term for which such servces would have otherwe ben provided to
the Customer to be imeditely due and payable.
(A) Upon nonpayment of any amounts owig to the Company, the Company maydicontiue or suspend servce without incurring any libilty. Company wil
comply with the Telephone Customer Relations Rule.
(B) Upon violation of any of the other material term or conditions for furnhigservice the Company may, diontiue or suspend servce without incurrg any
liabilty if such violation contiues during tht period.
(C) Upon condemntion of any material portion of the facilties used by the Company
to provide service to a Customer or if a casualty renders al or any materi porton
of such facilties inoperable beyond feasible repai, the Company, by notice to the
Customer, may dicontiue or suspend service without incurrig any liabilty.
(D) Upon the Customer's inolvency, assignent for the benefit of creditors, fig for
banptcy or reorgantion, or faig to discharge an involunta petition with
the tie permittd by law, the Company may imediately dicontiue or suspend
service without incuring any libilty. Company wi comply with Federal
Banptcy law and rules.
(E) Upon any governenta prohibition or governenta requied alteration of the
services to be provided or any violation of an applicable law or reguation, the
Company may imediately dicontiue servce without incuring any liabilty.

2.6.4 Notice to Company for Cacellation of Servce

2.6.5 Cacellation of Application for Servce
(A) Where the Company permts the Customer to cancel an application for servce pnor
to the start of service or pnor to any speia constrction, no charges wi be imposed
except for those specifed below.
(B) Where, prior to cancelltion by the Customer, the Company incurs any expenss in
intag the servce or in preparing to inta the service that it otherw would not
have incured, a charge equal to the costs the Company incued, less net salvage,
shal apply, but in no case shal th charge exceed the sum of the charge for the
mium period of services ordered, includig intation chages, and al charges
others levy agait the Company that would have be chargeable to the Customer
had servce begu

where speci arangements of facilties or equipment have begu, before the

2.6.6 Changes in Servces Requested
If the Customer maes or requests mateia changes in circut engieerig, equipment
specifcations, servce parameters, preITes locations, or otherwe matenay modies any
proviion of the application for servce, the Customet s intaation fee shall be adjusted
accordigly.
2.6.7 Bad Check Charge
A service charge of $20.00 wi be assesed for all checks retued by a ban or other
ficia intitution for: Inufcient or uncollecd fuds, closed account, apparent
tapering, ITSing signtue or endorsement, or any other inciency or direpancy
necessitatig retu of the inbuent at the diretion of the drawee ban or other ficial
intitution.
2.7 Allowances for Interrptions in Servce
2.7.1 General
Credit for Interruptions: When the use of service or facilties furnhed by the Company is
interrupted due to any cause other than the negligence or wil act of the Customer, or
the operation or faiure of the facilties or equipment provided by the Customer, pursuant
to IOAP A 31.41.01 Rules 502 and 503 subject to interrption wi be allowed for the
service and facilties rendered useless and inoperative by reason of the interruption,
whenever said interruption contiues for a period of 24 hours or more from the tie the
interruption is reported to or known to exist by the Company, except as otherwise
specifed in the Company's tariffs. If the Customer reports a service, facilty or circuit to
be inoperative but declies to release it for testig and repai, it is considered to be
impaied, but not interrupted.
For calculatig credit alowances, every month is considered to have 30 days. A credit
alowance is applied pursuant to IDAP A 31.41.01 Rules 502 and 503 based on the rates
specified hereunder for Local Line or Local Trunk Service and is dependent upon the
lengt of the interruption. Ony those facilties on the interrpted portion of the circuit
wil receive a credit. Credit allowances for service outages that exceed 24 hours in
duration wi be rounded up at the next whole 24 hours.

(C) If the Customer reports a servce, facity or circut to be interrpted but declies to

(D) The Customer shal be responsible for the payment of serice charges as set forth

Customer when the servce diculty or trouble report results from the use of
equipment or facilties provided by any party other than the Company, includig but
not lited to the Customer.
2.7.2 Limtations of Alowances
No credit alowance wi be made for any interrption in servce:
(A) interruptions due to the negligence of, or noncompliance with the provisions of th
ta by, the Customer, Authorized User, Joint-User, or other common carrier
providing service connected to the service of Company;
(B) interruptions due to the negligence of any person other than the Company
including, but not lited to, the Customer connected to the Company's facilties;
(C) interruptions due to the faiure or malfction of non-Company equipment;
(D) interruptions of service during any period in which the Company is not given fu
and free access to its facilties and equipment for the purpose of investigatig and
correctig interruptions;

(E) interruptions of service during a period in which the Customer contiues to use the
service on an impaied basis;
(F interruptions of service during any period when the Customer has released service
to the Company for maintenance purposes or for implementation of a Customer
order for a change in servce arrangements;
(G) interruption of service due to circumstaces or causes beyond the control of the
Company.
2.7.3 Use of Another Mean of Communcations
H the Customer elects to use another mean of communcations durg the period of
interruption, the Customer must pay the chges for the alternative service usd.
2.8 Cancellation of ServcEiermnation Liabilty
H a Customer cancels a servce order or termates servces before the completion of the term for any
reason other than a servce interrption (as defied in Seon 2.7.1) or where the Company breaches
the terms in the service contract Customer may be requested by the Company to pay to Company
termiation libilty charges, which are defied below. These charges shal beome due and owig
as of the effective date of the cancelltion or termation and be payable with the period set forth
in Setion 2.6.2.
2.8.1 Termnation Liabilty
Customer's termation libilty for cancellation of service shal be equal to:
(A) al unpaid non-recg charges reasonably expended by Company to establih
service to Customer, plus;
(B) any diconnecon, early cancellation or termation charges reasonably incured and
paid to thd pares by Company on behal of Customer, plus;
(C) al recg charges specifed in the applicable Servce Order for the balce of the
then cuent term dicounted at the prie rate anounced in the Wall Street Journal on
the thd business day followig the date of cancelation;
(D) mius a reasonable alowance for costs avoided by the Company as a diect result of
Customer's cancellation.

2.9 Transfers and Assignents
Neither the Company nor the Customer may assign or tranfer its rights or duties in connecon with
the servces and facilties provided by the Company without the wrttn consent of the other party,
except that the Company may assign its rights an duties to a) any subsidi, parent company or
afte of the Company; b) pursuant to any sale or tranfer of substatiy al the assets of the
Company; or c) pursuant to any fiancig, merger or reorganation of the Company.
2.10 Flexible Pricing
Notice to Customers of rate changes shall be made in accordance with Idaho Code 62-606. Where
there are no regulations, notiication wi be made in a maner appropriate to the circumstances
involved. A Customer can request that the Company disconnect servce that is provided under
the Flexible Pricing due to a price increase. The customer wi be credited for the diference
between the new price and the old price retroactive to the effective date of the price increase if the
customer notiies the Company of its desire to diconnect servce within 20 days of receiving
notication of the price increase.

Where charges for a servce are specifed based upon ditace, the followig rues apply:
5.3.1 Distace between two points is measured as ailie ditace between the rate centers of the
origiatig and termatig teephone lies. The rate center is a set of geographic
coordite, as referenced in Local Exchange Routig Guide issued by Bellcore, associated
with each NP A-NX combintion (where NP A is the area code and NX is the fist thee
digits of a seven-digit telephone number). Where there is not telephone number associated
with an access lie on the Companýs network (such as a dedicated 800 or WATS access
lie), the Company wi apply the rate center of the Customets mai bilg telephone
number.
5.3.2 The ailie ditace betwee any two rate centers is determed as follows:
Step 1: Obtain the uv" (vertcal) and "H" (horionta) coordite for each Rate Center
from the above-referenced Bellcore docuent.
Step 2: Computer the dierence between he uv" coordiate of the two rate centers; and
the diference between the two uH" coordite.
Step 3: Square each dierence obtaed in step (b) above.
Step 4: Add the square of the uv" dierence and the square of the "H" dierence

5.6 Standard Business Line
The Stadard Business Line provides a Customer with a single, anog, voice-grade telephonic
communcations chanel, which can be used to place or receive one cal at a tie. Stadard
Business Lies are provided for the connection of Customer-provided wig and single station sets
or facsime maches. An optiona per lie Huntig featue is avaiable for multi-lie Customers
which route a call to an idIe station lie in a prearanged group when the caled station lie is busy.
5.7 PBX Tru Servce
Basic PBX Tru Servce provides a Customer with a single, voice-grade telephonic communcations
chael, which can be used to place or receive one call at a tie. Basic Tru are provided for
connection of Customer-provided private branch exchanges (PBX) to the public switced
telecommuncations network. Each Basic PBX Trun is provided with touch-tone signg and may
be confgued into a hunt group at no additional charge with other Company-provided Basic PBX
Tru. The sign is an analog sign at the DS leveL.
5.8 Reserved for Futue Use
5.9 Direct Inward Dialng (DID) Servce
Direct Inward Dialg ("DID") permts cal incomig to a PBX system or other Customer Premis
Equipment to be routed to a speifc station without the assistace of an attndant. DID cal arerouted diectly to the station associated with the caled number. DID servce as offered by the
Company provides the necessar tr, telephone numbers, and out-puling of digits to enables
DID serice at a Customer's location. DID service requies specia PBX softare and hardware not
provided by the Company. Such hardware and softare is the responsibilty of the Customer.

(A) Flexible Ca Forwarding: Provides end-user control for cal forwardig capabilties vi
di-accessed voice prompt menus. Customers may forward cal to a pri local or
long ditace. The end-user may specif a secondary location for routig of go
unwered at the forward-to location or reach a busy sign Th secondary location
may be another telephone number, pager or voice messagig servce. Other capabilties
included with th feature include:
Speed Forwardig;
Priority Screenig;"
Rig Control; and
Timed Forwardig.
It is the responsibilty of the Customer to subscrbe to the telephone number, pager or
voice messagig servce usd as the seondar location.
(B) Cal Forwardig Don't Answer: Permts the forwardig of incomig cal when theend-user s lie remai unwered afr a pre-designted ringig interval. The
rigig interval before forwardig and the forward-to number are fied by the servce
order. However, the end-user has the abilty to tum the feature on or off at hi/her
discretion.
(C) Call Forwarding Busy Line: Permts the forwardig of incomig cal when the end-
users lie is busy. The forwarded number is fied by the end-user servce order.
However, the end-user has the abilty to tu the featue on or off at hi/her dicretion.

along with the forward-to number in order to tu the feature on. A separate code is
dialed by the end-user to deactivate the featue.
(F) Call Forwarding Varable, Remote Access: Permits the end-user to automaticaly
forward (tranfer) al incomig cal to another telephone number, and to restore it to
normal operation at their dicretion. The end-user must dia an activation code along
with the forward-to number in order to tu the feature on. A separate code is dialed
by the end-user to deactivate the featue. Featue activation may be performed from
the end-ust s exchange lie or remotely from some other lie. Remote access requies
the end-user to (1) di a speial access number 2) enter their seven-digit telephone
number and 3) enter a persona identication number prior to forwardig their cal.
(G) Thee Way Calling: Permts the end-user to add a thd party to an establihed
connection. When the thd party anwers, a two-way conversation can be held before
addig the origial party for a theeway conference. The end-user intiatig the
conference controls the cal and may disconnect the thd party to reestablih the
origial connection or establih a connection to a dierent thd par. The featue may
be used on both outgoing and incomig cal.
(H) Multiple Direcory Number Distictive Ringig: Th feature allows an end user to
determe the source of an incomig cal from a ditictive rig. The end user may
have up to two additiona numbers assigned to a single lie (i.e. Distictive Rigig -
First Number and Ditictive Rigig - Seond Number). The designated priar
number wi receive a normal rigig pattrn; other numbers wi receive ditictive
ringig pattern. The pattrn is based on the telephone number that the callg party
dia.

(N) Cal Tracing: Alows the tracing of nuiance cal to a specifed telephone number
suspected of origiatig from a given local offce. The tracing is activated upon
entering the specifed dial code. The origitig telephone numbe, outgoing tr
number or termtig number, and the tie and date are generated for every call to
the speced telephone number can then be identied.
(0) Call Block Alows the end-user to automaticaly block incomig cal from up to six
end-user pre-selected telephone numbers programed into the featue's screeg lit
Caers whose numbers have ben blocked wi hear a recorded message statig that
their cal has ben blocked. The end-user controls when the featue is active, and can

anwered or not. Upon activation, it wi redi the number automaticaly and contiue

When the Customer anwers the rig, the cal is then completed. The calg pars
number wi not be delivered or anounced to the cal recipient under any
circumtaces.
(Q) Anonymous Cal Rejecton: Permts the end-user to automaticaly reject incomig cal
when the call origite from a telephone number which has blocked deliery of its
callg number (see Calg Number Delivery Blockig). When active, call from
private numbers wi be routed to a specia anouncement then termated. The featue
may be tued on or off by the end-usr by dialg the appropriate featue control code.
Anonymous Cal Rejection is offered as a stad-alone featue or as an add-on to Caler
IDDeluxe.
(R) Callng Number Delivery Blockig: Prevents the delivery, diplay and anouncement
of the end-users Directory Number and Directory Name on al cal died from an
exchange servce equipped with th option. When active, the end-user s telephone
nae and number wi not appear on the caled pars Caler ID CPE or be disclosed in
another way. The featue is avaible on a per call or per lie basis. With per call
Calg Number Delivery Blockig, it is necessar for the end-user to di an actvation
code prior to placing the call. With the per lie version of the featue, all cal are
placed with the end-user s number blocked. Per lie end-users must di an actvation
code prior to utition.
(S) Message Waiting Indication: Provides the end-user with an audible (stuttr dil tone)
or visual (lamp or other CPE diplay) indication that messages are waitig to be
retreved. Message Waitig Indication can only be actvated/deactivated by a voice
maibox or other voice messagig servce provided by the Company or thd party. It is
the responsibilty of the Customer to subscribe to a compatible voice messagig service.
Visual Message Waitig Indication requies specaId CPE not provided by. the
Company. It is the responsibilty of the Customér to provide the necessar CPE.

For each Customer of Company-provided Exchange Servce(s), the Company shal arange for the
litig of the Customets ma bilg telephone number in the diectory(ies) publihed by the
domiant Local Exchange Carier in the area at no additional charge. At a Customet s option, the
Company wil arange for additional litigs for an additiona charge.
5.12.1 Non-Published Servce
Th optiona servce provides for suppression of prited and recorded diectory litigs. A
Customets name and number do not appear in prited diecories or Diectory Assistace
Bureau records.
5.12.2 Non-Listed Servce
Th optiona servce provides for suppression of prited diectory litigs only. Parties
may sti obta the Customet s number by calg the Directory Assistace Bureau.
5.13 Directory Assistance
Provides for identication of telephone directory numbers, via an operator or automated platform.
Customers are provided with a maximum of 2 litigs per each cal to Directory Assistace.
5.14 Miscellaneous Servces
5.14.1 Presubscrption Servces
Th servce provides for the Presubscription of local exchange lies provided by the
Company to the intraLATA and interLATA long ditace carrier(s) selected by the
Customer.
5.14.2 Pay Per Call BlockingUnblockig
Th servce provides the option of blockig, or subsequent unlockig, al 900 and 976 cal
on a per lie basis. The Company wi provide for per-lie blockig where the Companý s
switchig facilties permt.

A. Description
Th is an optiona arrangement avaible to cutomers with two or more individual
lie or tr servces. Where facities permt, such lies/tr wi be aranged so
that incomig cal to a busy lie/ tr wi overfow to other avaible
lies/ tr for that cutomer. The followig types of huntig arrangements are
avaible: series and multie (basic huntig), circu and preferenti.
B. Rates and Charges
1. The rate for each individual lie/ tr arranged for Huntig Service is in
addition to the regu access lie/ tr rate.
2. The nomecg charge applies for business cutomers to establih,
change to or from or to rearange Huntig Servce, except when chgig
from series to multie or vice versa.
3. The followig rates and charges are for each access lie/ tr aranged for
Huntig Servce. They are applicable to al but the last lie so aranged.

PBX Tru servce provides a Customer with a single, voice-grade telephoiuc communcations
chanel which can be usd to place one cal at a tie. Tru are provided for connection of
Customer-provided private branch exchanges (pBX) or other station equipment to the public
switced telecommuncations network.
PBX Tru are avaiable to Business Customers as Inward, Outward or Two-Way combination
tr where servces and facilties permL
Each PBX Tru is provided with Touchtone signg at no additiona chge. An optional per
tr Huntig featue is avaiable for Customers which routes a cal to the next idle tr in a
prearanged group (see Setion 7.3). .
PBX Tru may also be equipped with Direct Inward Diag (DID) capabilty and DID number
blocks for additiona charges (see Seon 7.6). Usage charges are lited in Setion 7.3.

7.6 Direct Inward Dialg (DID) Serce
Direct Inward Diag ("DID") permts cal incomig to a PBX system or other Customer Premies
Equipment to be routed to a specifc station without the assistace of an attdant DID cal arerouted diectly to the station associated with the caled number. DID servce as offered by the
Company provides the necessar tr, telephone numbers, and out-puling of digits to enable
DID service at a Customers location. DID servce requies speial PBX softare and hardware not
provided by the Company. Such hardware and softare is the responsibilty of the Customer.
The followig chges apply to Customers subscribing to DID service provided by the Company.
These charges are in addition to recring and non-recrrg charges for PBX Tru as shown in

9.1 ISDN PRI Servce with Unlimited Local Calling
ISDN PRI offers an array of value-added features, such as callg number identication and cal-
by-cal selection that enhance productivity. ISDN PRI is confgued with 23 64 Kbps bi-diectiona
B (Bearer) chanels and one 64 Kbps D (Data) chaneL. Unique to ISDN PRI is its abilty to
designate the D chanel to handle al of the signalg and call control requiements and leave the
remaig 23 B chanels free for any mi of circuit-switched voice and data. .
Each of these products is offered under a 12, 24 or 36 month term agreement. Rates include
unted local calg for sent-paid, diectly dialed call. Rates do not include calg card cal,
inormation tye cal to Time and Weather, 555, 700,900,976 Services, Directory Assistace or
any other tye of Operator Handled cal.
ISDN PRI includes the following non-optional Featue Package: Inbound Calg Line ID-Name &
Number and Call by Call Selection.
Regional Toll and Long Distace Services must be PIC'd to the Company. These rates are in
addition to ISDN PRI and DS1 rates below.

Th servce provides a trun side DS1 electrical interface from the cutomet s digita PBX system
to a digita port on a local Company switch for the origiation and termition of call. Trafc to

ISDN BRI (Basic Rate Interface) uses standard "twted pai" cables and is nearly thee ties
faster than a 56K dial up lie. ISDN PRI (Priary Rate Interface) uses a 1.54 Mbps digital
tranport facilty (Tl). Both servces provide the superior clarity of digita tranmision, a high-

11.1 Carier Presubscription
11.1.1 General
SECfON 11.0 - MISCELLANEOUS SERVICES
Carrier Presubscription is a procedure whereby a Customer designates to the Company
the carrier which the Customer wihes to be the carrier of choice for intraLATA and
interLATA toll call, Such cal are automaticaly directed to the designated carier,
without the need to use carrier access codes or additional dialig to direct the call to the
designated carrier. Presubscription does not prevent a Customer who has presubscribed
to an IntraLATA or InterLATA toll carrier from using carrier access codes or additional
dialg to diect call to an alternative long ditace carrier on a per cal basis.
11.1.2 Presubscription Options - Customers may select the same carrier or separate carriers for
intraLAT A and interLAT A long ditace. The followig options for long ditance

If a new Customer is unble to make selecon at the tie the new Customer places an order
to establih local exchange servce, the Company wi read a random litig of al avaiable
intraLATA and interLATA carers to aid the Customer in selection. If selection is sti not
possible, the Company wi inorm the Customer that he/she wi be given 90 calendar days
in which to inorm the Company of hi/her choice for priar toll carreres) free of charge.
Unti the Customer inorm the Company of hi/her choice of prim toll carrier, the
Customer wi not have access to long ditace servces on a presubscribed basis, but rather
wi be requied to di a carer access code to route al toll cal to the carrieres) of choice.
Customers who inorm the Company of a choice for toll carrier presubscription with the
90-day period wi not be assessed a service charge for the intial Customer request.
Customers of record may intite an intraLATA or interLATA presubscrption change at
any tie, subject to the charges specifed in 11.1.5 below. If a Customer of record inquies

SECTON 13.0 - PROMOTIONAL OFFRINGS / CONTRACT & ICB
13.1 Promotional Oferigs
The Company, upon notication to Commsion, may make promotional offerigs to its service
which may include waiving or reducing the applicable charges for the promoted service. The
promotional offerings may be lited as to the duration, the date and ties of the offering and
the locations where the offerings are made.
13.2 Contract Rates / Individual Case Basis (ICB) Arangements
Competitive pricing arrangements at negotiated rates may be fuhed on an individual case
basis (ICB) in response to request by customers to BLC, for proposal or for competitive bids.
Service offered under this taiff provision wi be provided to the Customer pursuant to contract.
Unless otherwise specifed, the regulations for such arrangements are in addition to the
applicable reguations and prices in other sections of th tarif. Specialied rates or charges wi
be made available to simarly situated Customers on a non-dicritory basis. Company wi
provide the Commsion with copies of any ICB contractual arrangements.
