TO PROVIDE LOCAL EXCHANGE NOTICE OF MODIFIED
TELECOMMUNICATIONS SERVICES.PROCEDURE

NOTICE OF APPLICATION
YOU ARE HEREBY NOTIFIED that on March 12, 2004, BG Enterprises, Inc. dba
Grizzly Telephone filed an Application for a Certificate of Public Convenience and Necessity to
provide resold local exchange services throughout the State of Idaho.
YOU ARE FURTHER NOTIFIED that Grizzly represents that it provides only basic
local exchange service and is non-facility based with no plans to build facilities in the future.
The Company also states that it has interconnection agreements with Qwest Corporation and
Verizon Northwest Inc.
YOU ARE FURTHER NOTIFIED that Grizzly markets its services to individuals
and businesses with challenged credit histories and services them by providing basic local
exchanges service without requiring a deposit.
YOU ARE FURTHER NOTIFIED that Grizzly is incorporated in the State of
Montana with offices located in Missoula Montana. The Company also has obtained a
Certificate of Existence from the Secretary of State for the State of Idaho.
YOU ARE FURTHER NOTIFIED that the Application states Grizzly has reviewed
the laws and regulations of this Commission governing local exchange telecommunications
services in Idaho and will provide service in accordance with all laws, rules and regulations, or
will request a waiver of those rules believed to be inapplicable.

YOU ARE FURTHER NOTIFIED that the Commission has determined that the
public interest may not require a formal hearing in this matter and will proceed under Modified
NOTICE OF APPLICATION

Procedure pursuant to Rules 201 through 204 of the Idaho Public Utilities Commission s Rules
of Procedure, IDAPA 31.01.01.201 through .204.
YOU ARE FURTHER NOTIFIED that any person desiring to state a position on this
Application may file a written comment in support or opposition with the Commission within
twenty-one (21) days from the service date of this Notice. The comment must contain a
statement of reasons supporting the comment. Persons desiring a hearing must specifically
request a hearing in their written comments. Written comments and reply comments concerning
this Application shall be mailed to the Commission and the Applicant at the addresses reflected

YOU ARE FURTHER NOTIFIED that all proceedings in this case will be held
pursuant to the Commission s jurisdiction under Title 61 of the Idaho Code and that the
Commission may enter any final Order consistent with its authority under Title 61.
YOU ARE FURTHER NOTIFIED that all proceedings in this matter will be
conducted pursuant to the Commission s Rules of Procedure, IDAP A 31.01.01.000 et seq.
ORDER
IT IS HEREBY ORDERED that the Application ofBG Enterprises, Inc. dba Grizzly
Telephone for a Certificate of Public Convenience and Necessity be processed by Modified
Procedure, IDAP A 31.01.01.201-204. Persons interested in submitting written comments in this
matter should do so no later than twenty-one (21) days from the service date of this Order.
