Ths Tarff contain the descriptions, reguations, serce stadads and rates applicable to
the fushig of local service and facilties for telecommuncations serces provided by
AMERICAN DIA TONE, INC., 2323 Curlew Road, Suite 7, Duned, Florida 34698.
This Tarff applies for serces fushed with the State of Idao. Ths Tarff is on file with
the Idaho Public Utilities Commssion, and copies may be inspected durg normal business
hours, at the Company's pricipal place of business.

TARF FORMT
A. Sheet Numberig - Sheet numbers appea in the upper right hand corner of the sheet.
Sheets are numbered sequentially. However, new sheets are occasionally added to the
Tarff. When a new sheet is added between sheets aleady in effect, a decimal is added.
For example, a new sheet added between sheets 14 and 15 would be 14.1.
B. Sheet Revision Numbers - Revision numbers also appear in the upper right corner of each
sheet. These numbers are used to determe the most curent sheet version on file with
the Commssion. For example, the 4th revised Sheet 14 cancels the 3rd revised Sheet 14.
Because of varous suspension perods, deferrals, etc. the Commssion follows in their
Tarff approval process, the most curent sheet number on file with the Commssion is
not always the Tarff sheet in effect. Consult the Check Sheet for the sheet curently in
effect.
C. Paragraph Numberig Sequence - There are six levels of paragrph codig. Each level of
coding is subservent to its next higher level:

D. Check Sheets - When a Tarff filing is made with the Commssion, an updated Check
Sheet accompanes the filing. The Check Sheet lists the sheets contaied in the Tarff,
with a cross-reference to the curent revision number. When new sheets are added, the
Check Sheet is chaged to reflect the revision. An asterisk (*) designates all revisions
made in a given filing. There wil be no other symbols used on this sheet if these are the
only changes made to it. The Tarff user should refer to the latest Check Sheet to find out
if a paricular sheet is the most curent on file with the Commssion.

Section 1 Defitions
Cerin terms used generally thoughout ths Tarff are defied below.
Advance Payment: Payment of all or par of a charge required before the sta of serce.
Call Waiting: Provides the customer with a burt oftone to indicate tht another call is waiting.
The second cal can either be anwered by flashig the switch-hook or hagig up the telephone
and being rug back by the caller.
Calling Number il: Provides the Cusomer with the caling par's telephone number. This
informtion wil be displayed only if the Customer provides the appropriate hadware. Such
hardware is not available from the Company.
Company: AMERICAN DIA TONE, INC. a Florida Corporation which is the issuer of ths
Tarff.
Custom Calling Package: An optional servce consisting of Call Waiting and Thee-Way Calling.
Customer: The peron or other entity tht ordered serce and is responsible for the payment of
chages and for compliance with the Company's Tarff regulations.
Due Date: The date on which a Customer payment is due to the Company.
Local Provider: A company that fushes exchage telephone servce.
Nonpublished Listing: A diectory listing which is neither prited in a diectory nor available
from diectory assistace.
Non-Recurg Charges: The one~time intial chages for serces includig, but not Limted to,
chages for intallation and special fees for which the Customer becomes liable at the time the
Servce Order is executed.
Recurg Chages: The monthy charges to the Customer for serces that continue for the
agreed-upon duration of the servce.
Service Order: The request for local exchange servces initiated by the Cutomer in a format
specified by the Company.
Services: The Company's telecommuncation serces offered to the Customer.
Set-Up Charge: A non-recurg charge to establish the Customer's account.

Station: Telephone equipment from or to which calls are placed.
Three-Way Callng: The Customer can sequentially call two other people and add them together
to mae up a three-way call.
Toll Activity Charge: Toll calls are blocked, when service is ordered.
Underlying carer: The underlying incumbent Local Exchange Carrer, or other alterntive
carer, which provides facilties and/or local exchange telephone servces to the Company for
resale to the Customer.
User: A Customer or any other person authorized by the Customer to use servce provided under
this Tarff.

Section 2 Regulations
2.1 Scope
The Company undertes to fush local exchage communcations servce in
connection with one~way and/or two-way inormtion tranmission between points
withi the State of Idao under the ter of ths Tarff.
Customer may use serces and facilities provided under this Tarff to obtain access to
servces offered by other serce provider. The Company is responsible under this Tarff
only for the serces provided herein, and it assues no responsibility for any servce
provided by any other entity.
2.2 Limtations
Servce is offered subject to the availability of underlyig carer facilties and the
provisions of this Tarff.
The Company reseres the right to discontinue fushig servce or limt the use of
servce necessitated by conditions beyond its control, or when .the Customer is using
servce in violation of the law or the provisions of ths Tarff.
2.3 Term and Conditions
Service is provided and biled on a prepaid basis, for a mium period of at least one
month and shall continue to be provided until caceled by the Customer, in writing or
verbally, on not less th th days notice. Unless otherwise specified herein, for the
purose of computing chages in ths Tarff, a month is considered to have 30 days. All
calculations of dates set fort in ths Tarff shal be based on, calenda days, uness
otherwise specified herein.
There is no billg of customers in area. The natue of the business of a prepaid local
exchange telephone serce is that the serce is terted uness payment is made in
advance of the provision of servce and the payment must be received by a certain date.

2.3 Term and Conditions (Continued)
At the expiration of any term specified in each Servce Order, or in any extension thereof,
servce shal continue on a month-to-month basis at the then curent rates uness termted
by either par. Any tertion shall not relieve Customer of his obligation to pay any
charges incured under the Servce Order and this Tarff prior to termation. The rights and
obligations, which by their natue extend beyond the termation of the term of the
Service Order shall surve such termation.
This Tarff shall be interpreted and governed by the laws of the State of Idaho without
regard for the State's choice of laws provision.
Another Telephone Company must not interfere with the right of any person or entity to
obta servce directly from the Company.
The Customer has no proper right to the telephone number or any other call number
designation associated with servces fushed by the Company. The Company reserves
the right to chage such numbers, or the central offce designation associated with such
numbers, or both, assigned to the Customer, whenever the Company deems it necessar
to do so in the conduct of its business.
2.4 Liabilty of the Company
The liability of the Company arsing out of the fushig of its serices includig but not
limted to mistakes, omissions, interrptions, delays, or errors, other defects, or
representations by the Company, or use of these servces or dages arsing out of the
failure to fush the servce whether caused by acts or omission, shall be limted to the
extension of allowances for interrption as set forth in Section 2.12 below, uness ordered
by the commssion. The extension of such allowances for interption shall be the sole
remedy of the Customer and the sole liability of the Company. The Company wil not be
liable for any direct, indirect, incidental, special, consequential, exemplary or puntive
daages to Customer as a result of any Company servce, equipment or facilties, or the
acts or omissions or negligence of the Company's employees or agents.
The Company's liabilty for wilfu misconduct, if established as a result of judicial or
admnistrative proceedgs, is not limted by ths Tarff. With respect to any clai or suit,

by a Customer or by any other, for dages associated with the orderig (including the
reservation of any specific number for use with a serce), intalation (includig delays
thereof), provision, tertion, matenace, repair, interrption or restoration of any
servce or facilities offered under ths Tarff, and subject to the provisions of Section
2.12, the Company's liability, if any, shal be limted as provided herein.

The Company shall not be liable for any delay or failure of performance or equipment
due to causes beyond its control, includig but not limted to: acts of God, fie, flood,
explosion or other catastrophes, any law, order, regulation, direction action or request of
the United States governent or of any other governent including state and loca
governents havig or claimig jursdiction over the Company, or of any deparment,
agency, commssion, bureau, corporation or other instrentality of anyone or more of
these federal, state, or local governents, or of any milita authority; preemption of
existing servce in compliance with national emergencies. riots, wars, unavailability of
right-of-ways or materials, or stries, lockouts, work-stoppages, or other labor
diffculties.
The Company shall not be liable for:
(a) Any act or omission of any entity. Furshig the Company or the Company's
Customers facilities or equipment used for or with the servces the Company offers,
or
(b) For the acts or omissions of other common carers or their employees or agents.
The Company shall not be liable for any damages or losses due to the fault or negligence
of the Customer or due to the failure or malfuction of Customer-provided equipment or
facilities.
The Customer shall indemnfy and hold the Company harmess from any and all loss,
claim, demds, suits or other action, or any liability whatsoever, whether suffered,
made, instituted, or asserted by any other pary or person(s), and for any loss, dage, or
destruction of any property, whether owned by the Customer or others, caused or claimed
to have been caused directly or indirectly by the intallation, operation failure to operate,
maintenance, removal, condition, location or use of any installation provided by the
Company. The Company reserves the right to require each Customer to sign an
agreement acknowledging acceptance ofthe provisions of ths Section as a condition to
such installations.

2.4 Liability of the Company (Continued)
The Company sha not be liable for any defacement of or dage to the Cutomer's
premises resulting from the fushig of serces or equipment on such premses or the
intalation or removal thereof, uness such defacement or dage is caused by the
wilfu misconduct of the Company's agents or employees. No agents or employees of
other entities shall he deemed to be agents or employees of the Company.
Notwithstandig the Customer's obligations as set fort in Section 2.9.2, the Company
shall be indemfied, defended, and held haress by the Customer or by others
authorized by it to use the servce againt any clai loss or daage arsing from
Customer's use or servces fushed under this Tarff, including:
(a) Clai for liable, stander, invasion of privacy or ingement of copyrght
arsing from the material, data, inormtion, or other content trmitted via the
Company's serce, and
(b) Patent ingement clai arsing from combing or connecting the service
offered by the Company with appartu and systems of the Customer or others,
and
(c) All other clai arsing out of any act or omission of the Customer or others;
in connection with any servce provided by the Company pursuat to ths Tarff.
The entire liability of the Company for any clai loss, dage or expense from any
cause whatsoever sha in no event exceed sum actuly paid to the company by the
Customer for the specific serces givig rise to the claim and no action or proceedig
agaist the Company sha be commenced more th one year after the servce is
rendered, uness ordered by the commssion.
The Company maes no warties or representations, expressed or implied either in fact
or by operation of law, statutory or otherise, includig waranties of merchatability or
fitness for a paricular use, except those expressly set fort herein.
The Company shal not be liable for any act or omission of any other company or
companes fushig a portion of the serce, or for dages associated with servce,
facilities or equipment which it does not fush, or for dages which result from the
operation of Cutomer-provided system, equipment, facilities or serces which are
interconnected with Company servces.

The Company does not guantee nor mae any warty with respect to servce
intallations at locations of which there is present atmosphere that is explosive, prone to
fire, dangerous or otherwise unuitable for such intalations. The Cutomer shall
indemnfy and hold the Company haress from any and all loss, clai, demds, suits
or other action, or any liabilty whatsoever, whether suffered made, instituted or asserted
by the Customer or by any other par, for any peronal injur to, or death of any person
or persons, or for any loss, damge or destruction of any propert, whether owned by the
Customer or others, caused or claimed to have been caused diectly or indiectly, by the
installation, operation, failure to operate, maintenace, removal, presence, condition,
locations or use of servce fushed by the Company at such locations.
The Company shall not be liable for the Customer's failure to fulfill its obligations to take
all necssar steps includin, without limtation, obtag, installing and maintainig all
necessar equipment, materals and supplies, for interconnecting the Station, termal
equipment or communcations system of the Customer or any thd-par acting as its
agent, to the underlyig carer's network. The Customer shal secure all licenes,
permts, rights of-way, and other arangements necessar for such interconnection. In
addition, the Customer shall inure that its equipment and/or system, or that of its agent,
is properly interfaced with the Company's service, that the signls emitted into the
underlyig carer's network are ofthe proper mode, band-width, power, data speed, and
signl level for the intended use of the Customer and in compliance with the criteria set
forth in Section 2.6 following, and that the signls do not dage Company or underlying
carer's equipment, injure its personnel or degrde servce to other Customers. If the
Customer or its agent fails to maintain and operate its equipment and/or system or that of
its agent properly, with resulting har to Company or underlyig carer's equipment,
personnel, or the quaity of servce to other Customers, the Company may require the use
of protective equipment at the Customer's. expene or termate the Customer's servce
without liability.

With respect to Emergency Number 911 Servce:
Ths servce is offered solely as an aid in handling assistace calls in connection
with fire, police and other emergencies. The Company is not responsible for any
losses, clai, demds, .suts or any liability whatsoever, wheter sufered, made,
instituted or asserted by the Cutomer or by any other par or person for any
personal injur or death of any person or persons, and for any loss, daage or
destruction of any property, whether owned by the Customer or others, caused or
claied to have been caused by;
Mistakes, omissions, interptions, delays, errors or other defects in the
provision of ths serce, or
Intalation, opertion, failure to operte, matenance, removal, presence,
condition, locaon or use of equipment and facilities fushig ths
servce.
Neither is the Company responsible for any ingement or invasion ofthe right of
privacy of any person or persons, caused or claimed to have been caused, directly
or indirectly, by the intallation, operation, failure to operate, maintenance,
removal, presence, condition, occasion or use of enhced 911 serce featues and
the equipment associated therewith or by any servces fushed by the Company
including, but not limted to, the identifcation of the telephone number, address or
name associated with the telephone used by the par or pares accessing enhanced
911 servce, and which arse out of the negligence or other wrongfu act of the
Company, the Cutomer, its user, agencies or muncipalities, or the employees or
agents of anyone of them.
The Company's liabilty arsing from errors or omissions in Directory Listings, other than
charged listings, shall be limted to the amount of actul impairent to the Customer's
service. In cases of chaged Directory Listings, the liability of the Company shall be
limted to an amount not exceeg the amount of chages for the chaged listings
involved durg the period covered by the diectory in which the eror or omission
occur.

2.4 Liability of the Company (Continued)
In conjunction with a Non-published Listing, as described in Section 3.4, the Company
will not be liable for failure or refusal to complete any call to such telephone when the
call is not placed by number. The Company wil tr to prevent the disclosure ofthe
number of such telephone, but will not be liable should such number be divulged.
When a Customer with a Non~published Listing as defied herein, places a call to the
Emergency 911 Servce, the Company wil release the name and address of the callig
part, when such inormation can be determed, to the appropriate local governental
authority responsible for the Emergency 911 Servce upon request of such
governental authority. By subscribing to servce under this Tarff, Customer
acknowledges and agrees with the release of inormation as described above.
2.5 Notification of Servce-Mfecting Activities
The Company wil provide the Customer reasonable notification of servce-affecting
activities that may occur in norml operation of its business. Such activities may
include, but are not limted to, Company or underlyig caer equipment or facilities
additions, removals or rearangements and routine preventative maintenance. Generally,
such activities are not specific to an individua Customer but affect may Customers
services. No specific advance notification is applicable to all servce activities. The
Company wil work cooperatively with the Customer to determe the reasonable
notifications requirements. With some emergency or unplaned service-affecting
conditions, such as outage resulting from cable damage, notification to the Customer
may not be possible.
2.6 Provision of Equipment and Facilities
The Company shall use reasonable efforts to mae available serces to a Customer on
or before a paricular date, subject to the provisions of and compliance by the Customer
with, the reguations contaied in this Tarff. The Company does not guantee
availability by any such date and shall not be liable for any delays in commencing
servce to any Customer.
The Company shall use reasonable efforts to cause the underlying carer to mainta its
own facilities which the Company fushes to the Customer. The Customer may not,
nor may the Customer pert others to, rearge, disconnect, remove, and attempt to
repair or otherise interfere with any of the facilities provided by the underlyig carer
or the Company.

2.6 Provision of Equipment and Facilties (Continued)
The Customer shall be responsible for the payment of servce charges as set forth herein
for visits by the Company's agents or employees to the Customer's premises when the
servce difficulty or trouble report results from the use of equipment or facilties
provided by any par other th the underlyig carer or the Company, including but
not limted to the Customer.
The Company shal not be responsible for the instalation, operation, or maintenance of
any Customer-provided communcations equipment. Where such equipment is
connected to the facilities fushed pursuant to ths Tarff, the responsibility ofthe
Company shall be limted to the fushig of facilities offered under ths Tarff and to
the maintenance and operation of such facilities. Beyond ths responsibilty, the
Company shall not be responsible for:
The transmission of signs by Customer-provided equipment or for the quality
of, or defects in, such tranmission; or
The reception of signs by Customer-provided equipment.
2.7 Ownership of Facilities
Title to all facilities provided in accordace with ths Tarff remains in the underlyig
carer, its agents or contractors.
2.8 Prohibited Uses
The servces of the Company shal not be used for any unawful purose or for any use
for which the Cutomer ha not obtaed all requied goverental approvals,
authoriations, licenses, consents and perts.

2.9 Obligations of the Customer
2.9.1 General
The Customer shall be responsible for:
A. The payment of all applicable chages pursuat to this Tarff;
B. Reimbursing the Company for dage to, or loss of the
Company's or underlyig carer's facilities or equipment caused
by the acts or omissions of the Customer; or the noncompliance
by the Customer with these regulations; or by fire or theft or
other casualty on the Customer's premises not caused by the
negligence or wilful misconduct of the employees or agents of
the Company. The Company wil upon reimbursement for
daages cooperate with the Customer in prosecuting a clai
againt the person causing such dage and the Customer shal
be subrogated to the Company's right of recovery of dages to
the extent of such payment;
C. Providing at no charge, as specified from time to time by the
Company, any needed personnel, equipment, space and power to
operate Company or underlyig carer facilities and equipment
intalled on the premises of the Customer;
D. Any costs associated with alterg the strctue to pert
installation ofthe underlyig carer-provided facilities. The
Company may require the Customer to demonstrate its
compliance with ths Section prior to accepting an order for
servce;
E. Granting or obtainig permssion for underlying carer or
Company agents or employees to enter the premises of the
Customer at any time for the purose of installig, inspecting,
maintaig, repairg, or upon teration of servces as
stated herein, removig Company or underlyig carer
facilities or equipment;

F. Not creating or allowing to be placed or matained any liens
or other encumbrances on the Company's or underlyig
carer's equipment or facilities.
2.9.2 Claims
With respect to any service provided by the Company, the Customer shall
indemnfy, defend and hold haess the Company from all clai, actions,
dages liabilties, costs and expenes, includig reasonable attorneys' fees for:
A. Any loss, destrction or dage to property of the Company or
any thid par, or the death of or injur to persons, including, but
not limted to, employees or invitees of either the Company or the
Customer, to the extent caused by or resulting from the negligent
or intentiona act or omission of the Customer, its employees,
agent representatives, or invitees; or
B. Any clai loss, dage, expense or liabilty for infrngement of
any copyrght, patent, trde secret, or any proprieta or
intellectu proper right of any third par, arsing from any act
or omission by the Cutomer, includig, without limtation, use of
the Company's serces and facilities in a maer not
contemplated by the agreement between the Customer and the
Company.

2.11 Payment Arangements
2.11.1 Payment for Servce
The Customer is responsible for payment of all charges for servce provided by
the Company to the Customer. If an entity other than the Company imposes
charges on the Company, in addition to its own internl costs, in connection with
a servce for which a Company Non-Recurng Chage is specified, those chages
may be passed on to the Customer.
Taxes & Fees
Federal, state, and any local taxes or fees (e.g., gross receipts tax, sales
tax, muncipal utilities tax) , if any, wil be added on the customer's bil
in addition to the rates listed in this price sheet.
Access Charges
Any access or other chages which are imposed by order of or at the
direction of the Federl Communcations Commssion wil be added on
the Customer's bil in addition to the rates listed in this price sheet.
2.11.2 Biling and Collection of Charges
Bils wil be rendered monthly to Customer.
All Non-Recurng Charges are due and payable in advance, uness other
mutully agreed argements were made in advance. All Recurg
Charges are due and payable upon request.
The Company shall present bils for Recurg Charges monthly to the
Customer in advance of the month for which servce is provided.
For Customers whose servce is disconnected there wil be refud in the
amount of whatever balance of the customers servce did not get used.
Amounts not paid by the Due Date are considered past due.
A late charge may be assessed on any past due balance.

The Customer shal notify the Company of any disputed item on a bill withi 30
days of receipt of the bilL. If the Customer and the Company are unable to resolve
the dispute to their mutu satisfaction, the Customer may file a complaint with
the Idao Public Utilties Commssion.
The date of the dispute shall be the date the Company receives suffcient
documentation to enable it to investigate the dispute. The date of the resolution is
the date the Company completes its investigation and notifies the Customer of
the disposition of the dispute.

2.11.5 Securty
Cutomer Deposits
There are no deposits requied of customers to establish servce. Serce is sold
on a prepaid basis in monthy increments.
2 11.6 Discontinuance of Servce
Discontinuance of serce wil be governed by the rues and reguations specified
in this Tarff.
Upon nonpayment of any regulated amounts owing to the Company, the
Company wil provide 5 days written notice prior to disconnection.
In the event the cusomer does not renew their servce with the company by not
mag a renewal payment prior to the end of their pre-paid period, servce wil
be discontinued without notice.
Upon violation of any of the other material term or conditions for fushig
servce the Company may discontinue or suspend serce without incurg any
liabilty if such violation continues durg that period.

The Company may discontinue the fushings of any and/or all servce(s) to a
Customer without incurg any liability,
Imedately and without notice, if the Company deems that such action is
necessar to prevent or to protect against fraud or to otherwise protect its or
the underlyig carer's personnel, agents, facilities or serces. The
Company may discontinue service puruant to ths subsection if:
A. The Customer refuses to fush inormation to the Company
regarding the Customer's credit~wortess, its past or curent use
of common carer communcations servces or its planed use of
servce(s); or
B. The Customer provides false information to the Company
regarding the Customer's identity, address, creditworthiess, past
or curent use of common carer communcations servces, or its
planed use of the Company's service(s); or
C. The customer states that it wil not comply with a request of the
Company for securty for the payment for servce( s) in accordace
with Section 2.1 1.5; or
D. Customer has been given 5 days written notice by the Company of
any past-due amount (which remains unpaid in whole or in par) for any
of the Company's other servces to which the Customer either subscribes,
or had subscribed or used.

D. The Customer uses or attempts to use, serice with the intent to
avoid the payment, either in whole or in par, of the Tarffed
charges for the serce by:
(1) Using or attempting to use servce by rearangig,
taperg with or makg connections to the
Company's serce not authoried by ths Tarff; or
(2) Using trcks, schemes, false or invalid numbers,
false credt devices, electronic devices; or
(3) Any other frudulent mea or devices; or
F. The Customer causes toll or any other charge by any entity other
than the underlying carer to appea on the Company's billing
from the underlying carer; or
G. The serce is abandoned by the Cutomer; or
H. The serce is used in such a maer as to interfere with the
serce of other user; or
i. The servce is used for unawfu puroses.
The suspension or discontinuace of servce(s) by the Company pursuant to ths
Section does not relieve the Customer of any obligation to pay the Company for
chages due and owing for serce(s) fushed durg the time of or up to
suspenion or discontinuace.

Upon the Company's discontinuace of serce to the Customer, all applicable
charges, includig termtion charges, shall become due. This is in addition to
all other remedies that may be available to the Company by law or in equity or
under any other provision of ths Tarff.
2.12 Allowances for Interrptions of Servce
2.12.1 Credit for Interrptions:
When the use of servce or facilities fushed by the Company is interrpted due
to any cause other tha the negligence or wilfu act of the Customer, or the
operation or failure of the facilities or equipment provided by the Customer, a pro
rata adjustment of the monthly Recurg Charges subject to interption will be
allowed for the servce and facilities rendered useless and inoperative by reason
of the interption whenever said interrption continues for a period over 48
hours or more from the time the interrption is reported to or known to exist by
the Company, except as otherwise specified in the Company's Tarffs. If the
Cutomer reports a servce to be inoperative but declines to release it for testing
and repair, it is considered to be impaired, but not interrpted.
Credit allowance for the interrption of servce which is not due to the
underlyig carer's testing or adjusting, negligence of the Customer, or
to the failure of chanels or equipment provided by the Customer, are
subject to the general liability provisions set forth in Section 2.4 herein.
It shall be the obligation of the Customer to notify the Company
imediately of any interrption in service for which a credit allowance
is desired. Before giving such notice, the Customer shal ascertain that
the trouble. is not being caused by any action or omission by the
Customer, with the Customer's control, or is not in wirg or
equipment, if any, fushed by the Customer and connected to the
Company's or underlying carer's facilties.

2.12.2 Limtations on Allowances
No credit allowance wil be made for:
A. Interrption due to the negligence of or noncompliance with
the provisions of this Tarffby the Customer, any User, or
other common carer providing servce connected to the
service of the Company if the ILEC's line is down.
B. Interrptions due to the negligence of any person other tha
the Company including, but not limted to, the Customer.
C. Interrptions due to the failure or mafuction of non-
Company equipment;
D. Interrptions of servce durg any period in which the
Company and the underlying carrer are not given free access
to their facilities and equipment for the purose of
investigating and correcting interrptions;
E. Interption of servce durg a period in which the Customer
continues to use the serce in an impaired basis;
F. Interrptions of servce durg any period when the Customer
has released serce to the company or underlyig carer for
maintenace puroses or for implementation of a Customer
order for a chage in servce arrgements;
G. Interrption of servce due to circumtaces or causes by
customer or users.

2.12.3 Use of Altertive Servce Provided by The Company:
Should the Customer elect to use an altertive servce provided by the Company
durg the period that a service is interrpted, the Customer must pay the Tarffed
rates and charges for the alternative servce used.
2.13 Cancellation of Servce
2.13.1 Cancellation of Application for Serce
Applications for serce are non-ccelable uness the Company otherwise
agrees. Where the Company perts the Customer to cacel an application for
servce prior to the sta of serce, no chages wil be imosed except for those
specified below.
Where, prior to cancellation by the Customer, the Company incurs any
expenses in installig the serce or in preparg to install the serce tht it
otherise would not have incured, a charge equal to the costs the Company
incured shall apply, but in no case shal this charge exceed the sum of the
chage for the mium period of servce ordered, including installation
charges, and al chages other levy againt the Company tht would have been
chageable to the Cuomer had serce begu.

2.13 Cancellation of Service (Continued)
2.13.2 Cancellation of Servce by the Customer
If a Customer cancels a Servce Order or terates servces before the
completion of the ter for any reason whatsoever other tha a servce
interrption (as defied in Section 2.12.1), the Customer agrees to pay to the
Company the following sum which shall become due and owing as of the
effective date of the cancellation or termation and be payable with the perod
set forth in Section 2.11.2: all costs, fees and expenes reasonably incured in
connection with:
A. Non-Recurg Charges reasonably expended by the
Company to establish serice to the Customer, plus
B. Any disconnection, ealy cancellation or tertion chages
reasonably incured and paid to thid paries by the Company
on behalf of the Customer, plus
C. Al Recurg Charges specified in the applicable Tarff of
the balance of the then curent term
2.14 Transfer and Assignents
Neither the Company nor the Customer may assign or tranfer its rights or
duties in connection with the servces and facilties provided by the Company
(or the underlyig carer) without the written consent of the other pary,
except that the Company may assign its rights and duties (a) to any subsidiar,
parent company or affliate of the Company; (b) pursuat to any sale or
transfer of any assets of the Company; or (c) pursuant to any fincing, merger
or reorganzation of the Company.
The Company will give customers notification of any tranfer assignents and
an option to change to another provider if the customer desires.

2.15 Notices and Communcations
2.15.1 The Company shall mail or deliver all notices and other communcations to the
Customer at the serce address, uness tht Cusomer designates a separate
address to which the Company's bills for serce shall be mailed. In the case
where a separte "billig address" ha been designted by the Customer, the
Company shall also mail or deliver all notices and other communcations to the
designated "biling address".
2.15.2 The Company shall designate on any Servce Order, and on each bill for servce,
an address to which the Customer shall mal or deliver all notices and other
communcations, except tht Company may designate a separte address on each
bill for serce to which the Customer sha mal payment on that bil.
2.15.3 All notices or other communcations requied to be given puruat to ths Tarff
wil be in wrting. Notices and other communcations of either par, and all bils
mailed by the Company, shall be presumed to have been delivered to the other
pary on the thid business day following deposit of the notice communcation or
bill with the U.S. Mail or a private delivery servce, prepaid and properly
addressed, or when actually received or refused by the addressee, whichever
occurs fist.
2.15.4 The Company or the Customer shall advise the other par of any changes to the
addresses designted for notices, other communcations or billig; by following
the proceures for givig notice set fort herein.

3.1 Local Exchange Servce
A Stadad Residence Line provides the Customer with a single, anog, voice-grde telephonic
communcations chael, which can be used to place or receive one cal at a tie. Stadad Residence Lines
are provided for the connection of Customer-provided wig and single station sets or facsimle machies.
The Company's local telephone service provides a Customer with the ability to connect to the
underlying carer's switchig network which enables the Customer to:
. place or receive calls to any callig Station in the local calling area, as
derined herein
. access enhanced 911 Emergency Servce (where available through the
underlying carrer);
. access operator services (as specified in Section 3.3);
. place call to toll free 8XX telephone numbers.
3.1.1 Servce Area
The Company's servce area incorporates all geographic regions-and exchanges
curently served by the following underlying carer:
Throughout the ILEC service areas with the State of Idaho.

The Company shall provide for a single directory listing in the telephone diectory

of the Customer. The listing wil also include the Customer's address.
B. Non-published Listings: A Non-published telephone number wil
be fushed at the Customer's request providing for the omission or
deletion of the Customer's telephone listing from the telephone directory

3.7 Emergency Servces (Enhanced 911)
Allows Customers to reach appropriate emergency services includig police, fire and
hospitaL. Enhced 911 has the ability to selectively route an emergency call to the
priar E9ll provider so tht it reaches the correct emergency servce located closest to
the caller. In addition, the Customer's address and telephone inormtion wil be
provided to the pri E91I provider for display at the Public Service Anwerig Point
(PSAP).

Section 5 Lifelie Progr
5.1 General
(A) The Lifeline program is designed to increase the availability of
telecommuncations services to low income subscribers by providing a
credit to monthy recurrng local servce for qualifyng residential
subscribers. Basic ters and conditions are in compliance with the FCC's
Order on Universal Servce in CC Docket No. 97-157, which adopts the
Federal-State Joint Board's recommendation in CC Docket No. 96-45,
which complies with the Telecommuncations Act of 1996.
(B) Lifeline is supported by the federal unversal serce support
mechansm.
(C) Federal baseline support of eight dollars and twenty-five cents
($8.25) is available for each Lifeline servce and is passed though to the
subscriber. An additional thee dollars and fifty cents ($3.50) credit is
provided by the Company. Supplemental federal support of one dollar and
seventy-five cents ($1.75), matchig one half of the Company
contrbution, wil also be passed along to the Lifeline subscrber. The total
Lifeline credit available to an eligible customer is thrteen dollar and fifty
cents ($13.50). The amount of credit wil not exceed the charge for local
servce, which includes the access line, the Subscriber Line Charge and
local usage.

(A) General
(1) Customers eligible under the Lifeline program are also
eligible for connection assistance under the Link-Up program.
(2) One low income credit is available per household and is
applicable to the priar residential connection only. The named
subscriber must be a curent recipient of any of the low income
assistance programs identified below.
(3) A Lifeline customer may subscribe to any local servce
offerig available to other residential customers. Since the Lifeline
credit is applicable to the priar residential connection only, it
may not be applied to multiple lines in a package for local serce.
(4) Toll blockig, if elected, wil be provided at no charge to
the Lifeline subscrber.

(5) No deposit wil be required of a Lifeline customer who
subscrbes to toll blocking. If a Lifeline customer removes toll
blocking prior to establishig an acceptable credit history, a
deposit may be required. When applicable, advance payments wil
not exceed the connection and local service charges for one month.
(6) The Federal Universal Servce Charge wil not be biled to
Lifeline customers.
(7) Lifeline subscrber's local serce will not be disconnected
for non- payment of regulated toll charges. Local service may be
denied for non-payment of local calls. Access to toll serce may
be denied for non-payment of regulated tolls.
(8) At no time shall a customer's Lifeline rate go below zero.5.3 Eligibility
(A) Customers are eligible if they paricipate in at least one of the following
programs:

5.4 Certification
(A) Proof of eligibility in any of the qualifyng low income assistance
programs should be provided to the Company at the time of application for
servces; or eligible Lifeline subscribers may enroll in the Lifeline
program by signing a document certifyg under penalty of peijur that
the customer paricipates in one of the Lifeline eligible programs and
identifyng the qualifyng program. When eligibility documentation is
provided subsequent to installation, the Lifeline credit wil be provided on
a going forward basis.
(B) The Company reserves the right to periodically audit its records, workig
in conjunction with the appropriate state agencies, for the purpose of
determining continuing eligibility. Information obtained durng such audit
wil be treated as confidential information to the extent required under
State and Federal laws. The use or disclosure of information concernng
enrollees wil be limited to purposes directly connected with the
administration of the Lifeline plan.
(C) When a customer is determined to be ineligible as a result of an audit, the
Company wil contact the customer. If the customer canot provide
eligibility documentation within 60 calendar days, the Lifeline credit will
be discontinued.

6..1 General
(A) Lin-Up is a program designed to increase the availabilty of

to the non-recurng installation and servce charges to qualifyng residential
subscribers. Basic ters and conditions are in compliance with the FCC's
Order on Universal Service in CC Docket 97-157, which adopts the Federal-

with the Telecommuncations Act of 1996.
(B) Link-Up is supported by the federal universal service support mechansm.
(C) A federal credit amount of fifty percent (50%) of the non-recurg charges
for connection of service, up to a maximum of thirty dollars ($30.00), is
available to be passed though to the subscriber.
6.2 Regulations
(A) General
(1) Customers eligible under Link-Up are also eligible for monthy
recurng assistance under the Lifeline program.
(2) Link-Up connection assistance is available per household and is
applicable to the primar residential connection only.
(3) The Link-Up credit is available each time the customer installs or
relocates the priar residential servce.
(4) To receive the credit, proof of eligibilty must be provided within
30 days after installation of servce.
(5) The total tarffed charges for connecting servce, including servce
and other installation charges, are considered in the credit
calculation.
