increase is deemed prudent and necessary to cover existing and ongoing expenses
associated with routine operation and allow a reasonable twelve percent (12%)
rate of return associated with ongoing operation.

3. Exhibit 19, Proposed CUSTOMER NOTICE for review and approval.

Hello, As per our conversation today, VP, Inc will file an amendment to its Application. The following information/documents need to be included with the amendment:
Cover letter – outlines what is being amended, i.e. tariff with new monthly rate with a reason for
the rate increase, a notice that will be sent to customers and identify when it’ll be sent (Rule 125 – Notice to Customers of proposed change in rates); Attach with the Cover Letter:
Updated Exhibit 14 – Tariff with one single rate of $55.00
A new Exhibit 19 – a copy of the notice to customer of proposed change in rates. I have attached a blank copy that needs to be submitted with the amendment to the Application and then sent customers.
