COMES NOW, Mayfeld Springs Water Company, Inc., (the "Company") by.and
through its counsel, Fisher Pusch & Alderman LLP and files its Petition to Alter cor Amend
Commission Final Order No. 30628 pursuant to Idaho Code § 61-624 and Commission Rule of
Procedure 326, IDAP A, 31.01.01.326. The Company requests that the Commission alter or
amend Order No. 30628 in one regard. Specifically, the Company requests that the Commission
set non-recurrng charges for reconnection to the water system after a disconnection resulting
from a customer default. In its Application and its Amended Application, the Company
requested that these non-recurrng charges be reviewed and set by the Commission; however,
Order No. 30628 did not address this request. Accordingly, as discussed in more detail below,
the Company asserts that there are good and suffcient reasons for altering or amending Order
No. 30628.

PETITION TO ALTER OR AMEND
On Februar 5, 2008 the Company filed its Application for a Certificate of Public
Convenience and Necessity. In the Application the Company requested authorization to charge
rates on an interim basis until final decision by the Commission. In its Proposed Interim Tarff
Schedule the Company requested that it be allowed to charge the following non-recurng
charges to reconnect to the system after a customer-caused disconnection from the system:
$35.00 Reconnection Fee during office hours, 8:00 am to 5:00 pm
Monday thr Friday except for the following holidays which fall on a
weekday: New Years Day, Memorial Day, 4th of July, Labor Day,
Thansgiving and Day after and Chrstmas Day.
$70.00 Reconnection Fee after office hours.
Proposed Interim Tarif(emphasis added). In its Amended Application filed on April 25, 2008,
the Company respectfully requested that the Commission approve these same non-recurng'
charges as set for Exhibit 8 which was attached thereto. Amended Application at p. 6; See also
attached Exhibit 8. In Final Order No. 30628 the Commission did not approve or disapprove of
these non-recurrng charges. See Order No. 30628.
The Company asserts that the amount it requests for these recurng charges is reasonable
based on its certified operator's (Valley Hydro, Inc.) hourly rate of$35.00 an hour for general
labor as is shown on the attached invoice. Exhibit 37. Each customer reconnection would not
only include the labor time on site to reestablish a connection but also the time for the certified
operator to have personnel travel to and from its office in Caldwell, Idaho to the Arbor Ridge
Subdivision that is south and west ofKuna, Idaho to perform this service.

CONCLUSION
Based on the foregoing, the Company respectfully requests that the Commission
approve the following non-recurrng charges for reconnection to the system after a
customer-caused disconnection:
$35.00 Reconnection Fee durng offce hours, 8:00 am to 5:00 pm
Monday thr Friday except for the following holidays which fall on a
weekday: New Years Day, Memorial Day, 4th of July, Labor Day,
Thansgiving, the day after Thansgiving and Chrstmas Day.
$70.00 Reconnection Fee after office hours.
